摘 要:目的:观察中山市横栏地区产超广谱β-内酰胺酶(ESBLs)菌株的药敏检测与分析。方法:对2019年3月-2020年10月在笔者所在医院门诊和住院患者的各种标本进行鉴定分离,得到菌株478株,对其进行产超广谱β-内酰胺酶菌株试验,应用纸片扩散法敏感试验(改良K-B法)进行药敏试验。评估肺炎克雷伯菌、大肠杆菌检出超广谱β-内酰胺酶试验结果。结果:478株病原菌检出产超广谱β-内酰胺酶菌株126株,检出率为26.4%;肺炎克雷伯菌240株,检出产超广谱β-内酰胺酶菌株82株,检出率为34.2%;大肠埃希菌238株,检出产超广谱β-内酰胺酶菌株44株,检出率为18.5%。肺炎克雷伯菌对常用抗菌药物的敏感性由低至高依次为妥布霉素、庆大霉素、复方磺胺甲恶唑、阿米卡星、环丙沙星、阿莫西林/克拉维酸、头孢酊、头孢哌酮/舒巴坦、亚胺培南;大肠埃希菌对常用抗菌药物的敏感性由低至高依次为复方磺胺甲恶唑、妥布霉素、环丙沙星、庆大霉素、头孢西丁、头孢哌酮/舒巴坦、阿莫西林/克拉维酸、阿米卡星、亚胺培南。结论:监测产超广谱β-内酰胺酶菌株情况可了解掌握横栏地区的细菌耐药趋势,对细菌的变迁规律进行动态监测,对临床细菌耐药减缓、防止细菌耐药、选择适宜抗菌药物具有重要的指导价值。Objective:To conduct the drug susceptibility detection and analysis of extended-spectrumβ-lactamase(ESBLs)bacterial strains in Henglan town.Method:Various specimens from outpatients and inpatients in our hospital from March 2019 to October 2020 were identified and isolated,and 478 strains were obtained,the test ofβ-lactamase producing strain was carried out.Drug susceptibility was completed by Kirby-Bauer disc agar diffusion method(K-B method),detection results of ESBLs from klebsiella pneumoniae and escherichia coli were assessed.Result:A total of 126 ESBLs bacterial strains were discovered(26.4%),including 82 ESBLs bacterial strains from 240 strains of Klebsiella Pneumoniae(34.2%)and 44 ESBLs bacterial strains from 238 strains of Escherichia Coli(18.5%).The susceptibility of Klebsiella Pneumoniae to common antimicrobial agents from low to high was Tobramycin,Genamicin,Cotrimoxazole,Amikacin,Ciprofloxacin,Amoxicillin/Clavulanic Acid,Cefoperazone/Sulbactam,Imipenem,the susceptibility of Escherichia Coli to common antimicrobial agents from low to high was Sulfamethoxazole,Tobramycin,Ciprofloxacin,Genamicin,Cefoxitin,Cefoperazone/Sulbactam,Amoxicillin/Clavulanate,Amikacin,Imipenem.Conclusion:The monitoring of ESBLs bacterial strains is conducive to knowing about the drug susceptibility change trends,and providing guidance on the dynamic examination of bacterial evolution rules,the reduction of antimicrobial resistance,and the selection of the proper antibacterial agents.
