基于量化评估下的早期四级康复训练对重症监护病房机械通气患者运动认知功能恢复及预后的影响

摘　　要：目的:探讨基于量化评估下的早期四级康复训练对重症监护病房机械通气患者运动和认知功能的恢复及预后的影响。方法:选取2018年12月-2020年7月本院重症监护病房实施机械通气的患者70例,作为研究对象,随机数字表法分成观察组与对照组,各35例。对照组行常规早期康复训练,观察组行基于量化评估下的早期四级康复训练。比较两组干预前后的肺功能指标、康复后运动和认知功能评分、康复后并发症发生率。结果:康复训练后,观察组FVC、PEF及FEV_(1)/FVC均高于对照组(P<0.05);观察组运动和认知功能评分均高于对照组(P<0.05);观察组并发症发生率8.57%,低于对照组的28.57%(P<0.05)。结论:临床对重症监护病房的机械通气患者治疗中,采用基于量化评估下的早期四级康复训练能够改善患者肺功能相关指标,提高治疗效果,能够增强患者的独立性,提高运功功能及认知功能,且可降低并发症发生率,安全性较高,值得推广。Objective:To explore the effect of early four level rehabilitation training based on quantitative evaluation on motor and cognitive function recovery and prognosis of patients with mechanical ventilation in ICU.Method:From December 2018 to July 2020,70 patients with mechanical ventilation in ICU of our hospital were selected as the research objects and randomly divided into the observation group and the control group,35 cases in each group.The control group received routine early rehabilitation training,and the observation group received early four level rehabilitation training based on quantitative evaluation.Pulmonary function indexes,motor and cognitive function scores after rehabilitation,incidence of complications after rehabilitation were compared between the two groups before and after intervention.Result:After rehabilitation training,FVC,PEF and FEV_(1)/FVC in the observation group were higher than those in the control group(P<0.05).The scores of motor and cognitive function in observation group were higher than those in control group(P<0.05).The incidence of complications in the observation group(8.57%)was lower than that in the control group(28.57%),the differences were statistically significant(P<0.05).Conclusion:In the clinical treatment of ICU patients with mechanical ventilation,early four-level rehabilitation training based on quantitative evaluation can improve the related indicators of lung function,improve the treatment effect,enhance the independence of patients,improve motor function and cognitive function,and reduce the incidence of complications,with high safety,which is worthy of promotion.
