摘  要:目的探讨血尿清胶囊治疗IgA肾病(IgAN)的疗效及作用机制。方法随机把80只SD大鼠分成空白组、模型组、血尿清胶囊高、中、低剂量组和肾炎康复片组。IgAN大鼠模型建立应用口服牛血清白蛋白(BSA)+尾静脉注射脂多糖(LPS)+皮下注射四氯化碳(CCl4)的方法。观察治疗前后大鼠一般状态,检测治疗前后各时相尿红细胞计数(RBC)、24 h尿蛋白定量(UTP)及治疗后肾组织中基质金属蛋白酶(MMP)-9,金属蛋白酶抑制因子(TIMP)-1含量。结果①造模开始后第6周,造模组大鼠出现精神倦怠,进食减少,乏力嗜睡,毛色晦暗,尿色加深,体重减轻,第9周验证IgAN动物模型成功后,给予药物治疗,各组大鼠上述症状明显改善;②给药后治疗组大鼠尿RBC及UTP降低,血尿清胶囊中剂量组变化更加明显(P<0.05),且优于肾炎康复片组(P<0.05);③给药后治疗组大鼠肾组织MMP-9含量均明显上升,TIMP-1含量均明显下降,MMP-9/TIMP-1明显上升,血尿清胶囊中剂量组变化更加明显(P<0.05),明显优于肾炎康复片组(P<0.05)。结论血尿清胶囊能够调节MMP-9/TIMP-1从而改善IgAN。Objective To investigate the therapeutic effect and mechanism of Xueniaoqing capsule on IgA nephropathy.Methods Eighty SD rats were randomly divided into blank group,model group,Xueniaoqing capsule high,medium and low group and nephritis rehabilitation tablet group.The method of oral BSA+tail vein LPS+subcutaneous injection of CCl4 was used to establish IgA nephropathy rat model.The general state of rats before and after treatment was observed.Urine RBC counts and 24-hour urine protein quantification(UTP)before and after treatment,and the content of MMP-9 and TIMP-1 in renal tissue after treatment were detected.Results From the 6th week,the rats in the model group showed mental listlessness,decreased food intake,fatigue and lethargy,dark hair color,deepened urine color,and weight loss.After drug treatment,the above symptoms were significantly improved.After administration,RBC counts and 24-hour UTP of rats in the treatment groups were decreased,and the changes of Xueniaoqing capsule medium dose group were more obvious(P<0.05),which were better than those in the nephritis rehabilitation tablet group.After administration,the content of MMP-9 in the kidney tissue of the treatment groups increased,TIMP-1 decreased,and MMP-9/TIMP-1 increased.The changes of Xueniaoqing capsule medium dose group were more obvious(P<0.05),which were significantly better than those in the nephritis rehabilitation tablet group(P<0.05).Conclusions Xueniaoqing capsule has the effect of reducing the RBC counts and 24-hour UTP in IgA nephropathy model rats.MMP-9 and TIMP-1 in renal tissue are involved in the pathogenesis of IgA nephropathy,and Xueniaoqing capsule could regulate MMP-9/TIMP-1 and improve IgA nephropathy.
