新疆和田地区维吾尔族瘦素、脂联素及其比值与代谢综合征的关系

摘　　要：目的研究新疆和田地区维吾尔族居民瘦素(LEP)、脂联素(ADP)及瘦素/脂联素比值(L/A)与代谢综合征(MS)的关系,并探讨其对MS的诊断价值。方法从新疆和田地区开展的膳食营养与肥胖现况调查资料中随机抽取180例,其中MS患者47例(MS组),非MS者133例(非MS组)。调查内容包括一般情况调查、体格测量及生化指标(血脂、血糖、LEP、ADP)。根据MS组分数量,将受试者分为0组分、1组分、2组分者及MS组,比较不同MS组分患者上述指标的差异,采用Pearson相关或秩相关分析LEP、ADP、L/A与MS及组分的相关性;根据受试者工作特征(ROC)曲线判断LEP、ADP、L/A对MS及组分的诊断价值。结果MS组肥胖、高血糖、高血压、高TG血症、低HDL-C血症发生率为97.9%、21.3%、57.4%、85.1%、63.8%,均高于非MS组(P均<0.01)。在0组分、1组分、2组分者及MS组中,随着MS组分数量增加,ADP水平降低,L/A水平升高(P均<0.05)。相关性分析显示,ADP与MS呈负相关(r=-0.15,P<0.05),L/A与MS呈正相关(r=0.21,P<0.05)。ROC曲线分析显示,LEP诊断MS的ROC曲线下面积(AUC)为0.53(95%CI为0.44~0.62,P=0.371),灵敏度为63.8%,特异度为52.6%;ADP诊断MS的AUC为0.60(95%CI为0.50~0.70,P=0.007),灵敏度为51.1%,特异度为78.2%;L/A诊断MS的AUC为0.63(95%CI为0.54~0.72,P=0.033),灵敏度为53.2%,特异度为72.9%。L/A、ADP预测MS的AUC、约登指数、特异度、阳性预测值、阴性预测值均高于LEP。结论新疆和田地区维吾尔族居民ADP与MS呈负相关、L/A与MS呈正相关,且ADP、L/A对MS的诊断价值高于LEP。Objective To explore the relationships of leptin(LEP),adiponectin(ADP),and leptin/adiponectin(L/A)ratio with metabolic syndrome(MS)in Uygur adults in Hotan Prefecture of Xinjiang,and to explore their diagnos⁃tic value for MS.Methods A total of 180 subjects were collected from the investigation data of the correlation study be⁃tween dietary nutrition and obesity,including 47 people with MS(MS group)and 133 people without MS(non-MS group).General survey,physical examination and biochemical indexes(blood lipids,blood glucose,LEP,ADP)were done on all subjects.According to the number of MS components,the non-MS group was subdivided into the groups 0,1,and 2,and the differences between the above indicators in each group were compared.Pearson correlation or rank correla⁃tion analysis was used to analyze the correlations between LEP,ADP,L/A and MS,and MS components.The diagnostic value of LEP,ADP and L/A ratio for MS and its components was judged according to the receiver operating curve(ROC).Results The incidences of obesity,hyperglycemia,hypertension,hypertriglyceridemia and hypo-HDL-cholesterolemia in the MS group were 97.9%,21.3%,57.4%,85.1%and 63.8%,which were higher than those in the non-MS group(all P<0.01).With the increase of MS components in the groups 0,1,2,and MS group,ADP decreased and and the L/A ratio increased(all P<0.05).The correlation results showed that ADP was negatively correlated with MS(r=-0.15,P<0.05),and L/A was positively correlated with MS(r=0.21,P<0.05).The area under curve(AUC)of LEP level in diag⁃nosis of MS was 0.53(95%CI=0.44-0.62,P=0.371),the sensitivity was 63.8%and the specificity was 52.6%;the AUC of ADP level in diagnosis of MS was 0.60(95%CI=0.50-0.70,P=0.007),the sensitivity was 51.1%,and the specificity was 78.2%;the AUC of L/A ratio in diagnosis of MS was 0.63(95%CI=0.54-0.72,P=0.033),the sensitivi⁃ty was 53.2%,and the specificity was 72.9%.The AUC,Youden index,specificity,positive predictive value and nega⁃tive predictive value of L/A and ADP in predicting MS were hi
