摘　　要：目的:探究钢针内固定在掌指骨骨折患者中的应用效果及对手功能的影响。方法:选取本院2016年1月-2020年3月的82例掌指骨骨折患者为研究对象,将其根据治疗方式的不同分为对照组(切开复位钢板内固定)32例和观察组(钢针内固定)50例。比较两组的骨折愈合时间、不良反应发生情况、术后不同时间的疼痛程度及手功能状态、手术前后的骨代谢指标表达水平。结果:观察组的骨折愈合时间显著短于对照组,不良反应发生率显著低于对照组,差异均有统计学意义(P<0.05)。术后3、7、14 d,观察组的疼痛程度均显著轻于对照组,差异均有统计学意义(P<0.05)。术后3、6个月,观察组的手功能优良率均显著高于对照组,差异均有统计学意义(P<0.05)。术前,两组的骨代谢指标表达水平比较,差异均无统计学意义(P>0.05);术后2、4周,观察组的骨代谢指标表达水平均显著高于对照组,差异均有统计学意义(P<0.05)。结论:钢针内固定在掌指骨骨折患者中的应用效果较好,且对手功能的改善作用更为突出,因此在掌指骨骨折患者中的应用价值更高。Objective:To investigate the application effect of steel nail internal fixation in the patients with metacarpal and phalangeal fractures and influence for hand function.Method:A total of 82 patients with metacarpal and phalangeal fracture in our hospital from January 2016 to March 2020 were selected as the research objects.According to the different treatment methods,the patients were divided into control group(open reduction plate internal fixation)32 cases and observation group(steel needle internal fixation)50 cases.The fracture healing time,the occurrence of adverse reactions,the degree of pain and the functional state of the hand at different time after operation,and the expression level of bone metabolism indexes before and after operation were compared between the two groups.Result:The time of fracture healing in observation group was significantly shorter than that in control group,the incidence of adverse reactions was significantly lower than that in control group,the differences were statistically significant(P<0.05).At 3,7 and 14 d after operation,the pain degree in the observation group was significantly lighter than that in the control group,the differences were statistically significant(P<0.05).At 3 and 6 months after operation,the excellent and good rates of hand function in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Before operation,there were no significant differences in the expression levels of bone metabolic indexes between the two groups(P>0.05).2 and 4 weeks after operation,the expression levels of bone metabolism indexes in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The application effect of steel needle internal fixation in patients with metacarpal and phalangeal fracture is better,and the improvement effect of hand function is more prominent,so the application value in patients with metacarpal an
