双源CT冠状动脉CT血管造影在冠状动脉病变诊断中的价值分析

摘　　要：目的:探讨双源CT冠状动脉CT血管造影(CCTA)在冠状动脉病变诊断中的价值。方法:回顾性分析2018年2月-2020年4月本院接诊的89例疑似冠状动脉病变者的临床资料,均行CCTA检查,并以冠状动脉造影(CAG)检查结果为金标准,分析CCTA对冠状动脉病变的诊断价值。结果:89例患者经CAG共检出钙化斑块469个,Ⅰ、Ⅱ、Ⅲ、Ⅳ型钙化斑块分别为167、118、90、94个。以CAG为金标准,CCTA诊断冠状动脉钙化斑块准确率为91.47%(429/469),两者比较差异有统计学意义(P<0.05)。CCTA对Ⅰ、Ⅱ型钙化斑块检出率与CAG比较,差异均无统计学意义(P>0.05);CCTA对Ⅲ、Ⅳ型钙化斑块检出率均低于CAG(P<0.05)。结论:CCTA能较为准确地显示冠状动脉病变,其中对Ⅰ、Ⅱ型钙化斑块诊断价值较高,但对Ⅲ、Ⅳ型钙化斑块检出率较低,必要时可联合CAG诊断,以提高诊断准确率。Objective:To investigate the value of dual source CT coronary angiography(CCTA)in the diagnosis of coronary artery disease.Method:The clinical data of 89 patients with suspected coronary artery disease admitted to our hospital from February 2018 to April 2020 were retrospectively analyzed.All patients underwent CCTA examination,and the diagnostic value of CCTA for coronary artery disease was analyzed based on the results of coronary angiography(CAG)examination as the gold standard.Result:A total of 469 calcified plaques were detected in 89 patients by the CAG,Ⅰ,ⅡⅢ,Ⅳcalcified plaque was respectively 167,118,90,94.Using CAG as the gold standard,the accuracy rate of CCTA in diagnosis of coronary artery calcification plaque was 91.47%(429/469),and the difference between the two was statistically significant(P<0.05).There were no significant differences between CCTA and CAG in the detection rates of typeⅠand typeⅡcalcified plaques(P>0.05).CCTA were lower than CAG in the detection rates of typeⅢand typeⅣcalcified plaques(P<0.05).Conclusion:CCTA can accurately display coronary artery lesions,of which theⅠ,Ⅱcalcified plaque diagnostic value is higher,but theⅢ,Ⅳcalcified plaque detection rate is low,can joint the CAG diagnosis when necessary,in order to improve the diagnostic accuracy.
