摘 要:目的了解三甲公立医院医护人员病人安全文化感知现状,分析其影响因素,为改善我国医护人员病人安全感知、推动分级诊疗实践提供针对性建议。方法采取整群分层抽样方法在黑龙江省随机抽取3家三甲公立医院医护人员作为研究对象,针对医护人员人口学特征、医院病人安全文化感知等方面,进行问卷调查和数据分析。结果医护人员在"医院科室内团队合作""积极学习与持续改进"层面表现较好,积极反应率大于75%,而在"对错误的非惩罚反应""不良事件上报频率""人员配置""沟通的公开性"层面表现较不好,积极反应率小于50%。影响医护人员病人安全文化的因素有月薪、每月加班数、科室、工作年限、教育程度、职业等。结论基于分级诊疗背景下,三甲医院管理者应该开展在单位内部开展多样的教育培训活动、关注医护人员工作环境,积极举办丰富多样的内部活动,积极探索改善医护人员病人安全感知现况的助推路径,促进形成区域内医疗机构间重视病人安全文化的执业氛围。Objective To understand the current situation of patient safety culture perception of medical staff in public tertiary A hospitals in China,and analyze its influencing factors,so as to provide targeted suggestions for improving patient safety perception of medical staff in China and promoting hierarchical diagnosis and treatment practice.Methods A cluster stratified sampling method was adopted to randomly select medical staff from three public third-class A hospitals as the research objects,and a questionnaire survey and data analysis were conducted on medical staff’s demographic characteristics,hospital patient safety culture perception and other aspects.Results In"hospital departments team work""active learning and continuous improvement",medical staff perform well,and the positive response rate is greater than 75%.However in"response to the mistake of punishment""frequency of adverse events reported""personnel allocation""openness of communication"medical staff don’t perform well,and positive response rate is less than 50%.On this basis,it is found that the factors influencing the safety culture of medical staff and patients include monthly salary,monthly overtime work,departments,working years,education level,and occupation,etc.Conclusion Based on hierarchical diagnosis and treatment,managers of tertiary A hospital should develop various education training in units,pay attention to work environment of medical staff,actively hold various internal activity,and actively explore the boosting pathway in improving patient safety perception of medical staff,in order to form practice atmosphere of patient safety culture of medical institutions within the region.
