摘 要:目的分析河北省2017-2018年度流感样病例(ILI)流感病毒型别及其对神经氨酸酶抑制剂(NAI)的耐药性。方法2017年4月-2018年3月在河北省38家哨点医院收集ILI,采用Real-time PCR检测ILI咽拭子标本各型流感病毒核酸,分析检测阳性率。采用神经氨酸酶抑制试验检测部分毒株对奥司他韦和扎纳米韦的耐药性(计算半数抑制浓度IC50值)。结果河北省2017-2018年度哨点医院ILI流感病毒核酸检测总阳性率为17.50%(3707/21188),其中A(H1N1)、A(H3N2)、B-Victoria、B-Yamagata、H7、混合型流感病毒阳性率分别为7.44%、3.39%、0.40%、6.19%、0.005%、0.07%。1株A(H3N2)、7株A(H1N1)和28株B-Yamagata亚型流感病毒对奥司他韦的平均IC50分别为0.11、0.30(0.19-0.66)和13.93(4.54-32.07)nM;对扎那米韦的平均IC50值分别为0.30、0.20(0.07-0.25)和1.38(0.14-5.61)nM。结论河北省2017-2018年度流感病毒优势流行株为A(H1N1)和B-Yamagata亚型;流感病毒流行株对NAI类药物敏感。Objective To determine subtypes of influenza viruses isolated from influenza-like illness(ILI) cases during the 2017-2018 season in Hebei province and assess viral resistance to neuraminidase inhibitors(NAIs).Methods We obtained throat swabs from ILI cases in 38 sentinel hospitals of Hebei from April 2017 to March 2018. We used real-time PCR to detect nucleic acid for determination of positivity for influenza virus and influenza virus subtypes. We used a neuraminidase inhibition assay to test for resistance to oseltamivir and zanamivir(reporting half-maximal inhibitory concentrations, IC50 values).Results Among ILI cases in the 2017-2018 season in Hebei, 17.50%(3 707/21 188) were positive for influenza virus nucleic acid. Positivity for A(H1 N1), A(H3 N2), B-Victoria, B-Yamagata, H7, and mixed subtypes were 7.44%, 3.39%, 0.40%, 6.19%, 0.005%, and 0.07%, respectively. For one A(H3 N2) strain, seven A(H1 N1) strains, and 28 B-Yamagata strains, the average IC50 values to oseltamivir were 0.11, 0.30(0.19-0.66), and 13.93(4.54-32.07) nM;average IC50 values to zanamivir were 0.30, 0.20(0.07-0.25), and 1.38(0.14-5.61) nM.Conclusions The predominant influenza virus subtypes were A(H1 N1) and B-Yamagata during the 2017-2018 season in Hebei. Circulating influenza virus strains were sensitive to NAIs.
