摘　　要：当前,县域医共体建设仍存在科学的利益分配机制尚未建立、统一的人财物管理难以落实、基层机构资源配置不足、双向转诊与激励考核机制不完善等问题。按人头总额预付下,医共体要实现利益共享、成本控制与质量控制,就必须加强内部纵向协同。基于纵向协同的关键要素--组织管理、资源、服务与利益,要建立健全医共体利益分配机制,下放医共体管理权限,加大投入,改革双向转诊与绩效考核机制,实现医共体内部各级医疗机构之间关键要素的纵向协同发展。At present,there are still some problems in the construction of county medical alliances,such as the scientific benefit distribution mechanism has not been established,the unified management of human and property is difficult to implement,the resource allocation of grass-roots organization is insufficient,and the two-way referral and incentive assessment mechanism is not perfect.In order to realize benefit sharing,cost control and quality control,the medical alliances must strengthen the internal vertical collaboration under the global per capita budget.Based on the key elements of vertical collaboration,such as organizational management,resources,services and interests,it is necessary to establish and improve the benefit distribution mechanism of the medical alliances,devolve the management authority of the medical alliances,increase investment,and reform the two-way referral and performance evaluation mechanism,so as to achieve the vertical collaborative development of key elements among medical institutions at all levels within the medical alliances.
