TCT、HPV和阴道镜在门诊宫颈癌筛查中的应用

摘　　要：目的探讨新柏氏液基细胞学检测(TCT)、人乳头状瘤病毒(HPV)和阴道镜在门诊宫颈癌筛查中的应用效果。方法选取2019年1—12月在该院接受宫颈癌筛查的2000例疑似患者,均以手术病理检查结果作为金标准,接受TCT、HPV以及阴道镜活检,分析比较单独使用TCT、HPV以及联合TCT、HPV与阴道镜3种方式宫颈癌检查准确情况。结果2000例疑似患者中共有1307例患者确认,其中524例为CINⅠ、449例为CINⅡ、332例为CINⅢ、2例为浸润癌,HPV筛选结果为CINⅠ508例(96.95%)、CINⅡ432例(96.21%)、CINⅢ315例(94.88%)、浸润癌1例(50.00%);TCT筛选结果为CINⅠ476例(90.84%)、CINⅡ405例(90.20%)、CINⅢ298例(89.76%)、浸润癌1例(50.00%);3种技术联合筛选结果为CINI524例(100.00%)、CINⅡ449例(100.00%)、CINⅢ332例(100.00%)、浸润癌2例(100.00%),3种技术联合筛选结果的诊断准确率高于单独检查,差异有统计学意义(P<0.05)。结论在门诊宫颈癌筛查中采用采用TCT、HPV联合阴道镜,可及早发现患者病情情况,方便医护人员及时采取有效措施治疗。Objective To investigate the application effect of new Perch's liquid-based cytology test(TCT),human papilloma virus(HPV)and colposcopy in outpatient cervical cancer screening.Methods Selected 2000 suspected patients who were screened for cervical cancer in the hospital from January to December 2019.All of them used surgical pathological examination results as the gold standard,and received TCT,HPV and colposcopy biopsy.The analysis and comparison of the use of TCT,HPV and combined TCT,HPV and colposcopy three methods to check the accuracy of cervical cancer.Results A total of 1307 patients were confirmed in 2000 suspected patients,of which 524 were CINⅠ,449 were CINⅡ,332 were CINⅢ,and 2 were invasive carcinoma.The HPV screening results were CINⅠ508(96.95%)and CINⅡ432(96.21%),315 cases of CINⅢ(94.88%),1 case of invasive carcinoma(50.00%);TCT screening results were 476 cases of CINⅠ(90.84%),405 cases of CINⅡ(90.20%),298 cases of CINⅢ(89.76%),and 1 case of invasive carcinoma(50.00%);the combined screening results of the three techniques were CINⅠ524 cases(100.00%),CINⅡ449 cases(100.00%),CINⅢ332 cases(100.00%),and 2 cases of invasive carcinoma(100.00%).The diagnostic accuracy of the combined screening results of the three techniques was higher than that of individual examinations,and the differences was statistically significant(P<0.05).Conclusion The use of TCT and HPV combined with colposcopy in the outpatient cervical cancer screening can detect the patient's condition early and facilitate the medical staff to take effective measures in time.
