加味当归六黄汤联合甲巯咪唑片治疗阴虚火旺型甲状腺功能亢进症临床研究

摘　　要：目的:观察加味当归六黄汤联合甲巯咪唑片治疗阴虚火旺型甲状腺功能亢进症临床效果。方法:将140例阴虚火旺型甲状腺功能亢进患者,按照数字表法随机分为对照组和观察组各70例。对照组给予甲巯咪唑片治疗,观察组给予加味当归六黄汤联合甲巯咪唑片治疗,对比分析2组治疗前后甲状腺激素水平变化、症状评分及生活质量情况。结果:治疗后,2组中医主症、次症评分及总分均较治疗前降低(P<0.05),且观察组各项评分均低于对照组(P<0.05)。治疗后,2组游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)水平均较治疗前降低(P<0.05),促甲状腺激素(TSH)水平较治疗前升高(P<0.05);且观察组各项指标改善较对照组更显著(P<0.05)。治疗后,2组躯体功能、社会功能、物质生活、心理功能等生活质量各维度评分及总分较治疗前提高(P<0.05),且观察组各项评分均高于对照组(P<0.05)。结论:加味当归六黄汤联合甲巯咪唑片治疗阴虚火旺型甲亢疗效显著,可降低FT3、FT4水平,改善临床症状,提高生活质量。Objective: To observe the clinical effect of the therapy of modified Danggui Liuhuang tang combined with methimazole tablets on hyperthyroidism of the type of yin deficiency resulting in vigorous fire. Methods:A total of 140 cases with hyperthyroidism of the type of yin deficiency resulting in vigorous fire were randomly divided into the control group and the observation group according to number table method, 70 cases in each group. The control group was treated with methimazole tablets,and the observation group was additionally treated with the therapy of modified Danggui Liuhuang tang combined with methimazole tablets. The changes of levels of thyroid hormone, symptom scores and quality of life were compared between the two groups before and after treatment. Results:After treatment,the scores of main symptoms,secondary symptoms and the total score in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05). After treatment,free triiodothyronine(FT3) and free thyroxine(FT4) levels in both groups were decreased when compared with those before treatment(P < 0.05), and the thyroid-stimulating hormone(TSH) levels in both groups were increased(P < 0.05);the improvement of the above levels in the observation group was more significant than that in the control group(P<0.05). After treatment,the scores of physical function,social function,material life,mental function and total scores in the two groups were improved when compared with those before treatment(P<0.05),and the above scores in the observation group were higher than those in the control group(P<0.05). Conclusion:The therapy of modified Danggui Liuhuang tang combined with methimazole tablets has a significant therapeutic effect on the treatment of hyperthyroidism of the type of yin deficiency resulting in vigorous fire,which can reduce FT3 and FT4 levels,and improve clinical symptoms and the quality of life.
