摘 要:目的:研究自噬相关基因5(ATG5)和基质金属蛋白酶9(MMP9)在子宫内膜异位症(EMT)患者在位内膜、异位内膜的表达情况及两者表达的相关性。方法:收集安徽医科大学第二附属医院妇科病区2018年1月至2019年12月确诊的EMT患者的分泌期在位内膜与异位内膜35例为EMT组,同期收集正常分泌期子宫内膜组织35例为对照组,采用免疫组化方法检测并比较ATG5和MMP9在分泌期正常子宫内膜、EMT患者在位内膜与异位内膜及不同EMT分期患者(Ⅰ~Ⅱ期、Ⅲ~Ⅳ期)中的表达情况;采用Pearson相关分析ATG5与MMP9表达的相关性。结果:(1)EMT组患者在位内膜、异位内膜中ATG5的表达显著低于对照组(P<0.05),同时异位内膜中ATG5的表达低于在位内膜(P<0.05);EMT组患者在位内膜、异位内膜中MMP9的表达显著高于对照组(P<0.05),同时异位内膜中MMP9的表达显著高于在位内膜(P<0.05)。(2)Ⅰ~Ⅱ期EMT组患者在位内膜、异位内膜中ATG5的表达显著高于Ⅲ~Ⅳ期(P<0.05),而MMP9的表达显著低于Ⅲ~Ⅳ期(P<0.05)。(3)ATG5与MMP9的表达呈负相关(r=-0.850,P<0.05)。结论:ATG5的缺失和MMP9的激活均可能参与EMT的发生发展,ATG5和MMP9联合检测对临床上EMT的预测可能具有一定作用。Objective:To study the expressions and correlation of autophagy related protein 5(ATG5) and matrix metalloprotein 9(MMP9) in eutopic and ectopic endometrium from women with endometriosis(EMT).Methods:A total of 35 EMT patients who were admitted in our hospital from January 2018 to December 2019 were enrolled and served as the EMT group.The eutopic and ectopic endometrium in secretory phase were collected.Meanwhile, a total of 35 secretory phase normal endometrium were collected as control group.The expression of ATG5 and MMP9 in normal endometrium in secretory phase, eutopic endometrium and ectopic endometrium in EMT patients and patients with different EMT stages(stage Ⅰ-Ⅱ,stage Ⅲ-Ⅳ) were detected and compared by immunohistochemistry, the correlation between ATG5 and MMP9 expression was analyzed by Pearson correlation.Results:(1)The expression of ATG5 in the ectopic and eutopic endometrium of EMT group was significantly lower than that in the normal endometrium group, and the expression of ATG5 in ectopic endometrium was significantly down-regulated compared to that in the eutopic endometrium(P<0.05).The expression of MMP9 in the ectopic and eutopic endometrium of EMT group was significantly higher than that in the normal endometrium group, and the expression of MMP9 in ectopic endometrium was significantly up-regulated compared to that in the eutopic endometrium(P<0.05).(2)The expression of ATG5 in the ectopic and eutopic endometrium of Ⅰ~Ⅱ stage EMT was significantly higher than that in the Ⅲ~Ⅳ stage EMT,and the expression of MMP9 was significantly lower than that in the Ⅲ~Ⅳ stage EMT(P<0.05).(3)There was a negative correlation between ATG5 and MMP9 expression(r=-0.850,P<0.05).Conclusions:The absence of ATG5 and activation of MMP9 may be involved in the development of endometriosis.The combination testing of ATG5 and MMP9 could be beneficial for the prediction of EMT in clinical practice.
