摘　　要：目的:探讨不孕女性中,慢性子宫内膜炎(CE)的分布及诊断。方法:回顾性分析了2019年3月至2020年6月在成都西囡妇科医院就诊的411例不孕女性,根据子宫内膜组织跨膜硫酸已酰肝素蛋白聚糖合癸聚糖-1(CD138)免疫组织化学分析(IHC)染色结果分为CD138+组和CD138-组,其中CD138+诊断为CE。比较两组间患者的基础资料、宫腔形态特征和伊红(HE)染色结果。结果:CD138 IHC染色CE检出率高于HE染色(52.1%vs 38.0%,P<0.05)。宫腔镜下子宫内膜形态在CD138+组和CD138-组差异无统计学意义(P>0.05);分泌期子宫内膜和HE阳性是影响CD138结果的独立影响因素(OR 5.25,95%CI 1.37~20.04;OR 4.99,95%CI 3.09~8.07,P<0.05)。结论:CD138 IHC染色可以提高CE的检出率;宫腔镜检查不能独立诊断CE。Objective:To investigate the distribution and diagnosis of chronic endometritis(CE) in infertile women.Methods:A total of 411 infertile women were retrospectively analyzed during March 2019 and June 2020 in our hospital and divided into CD138+group and CD138-group according to CD138 IHC staining of endometrial tissue.Clinical data, endometrial morphology and the results of HE were compared between the two groups.Results:The detection rate of CE by CD138 IHC staining was higher than that of HE staining(52.1% vs.38.0%,P<0.05).Endometrial morphological results of hysteroscopy showed no significant difference between CD138+group and CD138-group(P>0.05).The results of logistic regression analysis confirmed that secretory phase endometrium(OR 5.25,95%CI:1.37~20.04,P<0.05) and HE-positive(OR 4.99,95%CI:3.09~8.07,P<0.05)were independent factors effect CD138 results.Conclusions:CD138 immunohistochemical staining couldimprove the CE diagnosis rate.Hysteroscopy cannot be used for diagnosing CE independently.
