侧卧位微创经皮肾镜碎石术在肾结石治疗中的效果观察及价值体会

摘　　要：目的:分析侧卧位微创经皮肾镜碎石术(PNCL)在肾结石治疗中的应用效果。方法:2018年8月-2020年8月收治肾结石患者50例,随机分为两组,各25例,均给予PNCL手术治疗。对照组取俯卧位;试验组取侧卧位。比较两组手术指标水平、穿刺成功率、Ⅰ期结石清除率及并发症发生率。结果:试验组手术时间、建立通道时间、下床时间均短于对照组,试验组术中出血量、碎石取石总时长均低于对照组,试验组穿刺成功率、Ⅰ期结石清除率均高于对照组,差异有统计学意义(P<0.05);两组手术时间与并发症发生率比较,差异无统计学意义(P>0.05)。结论:侧卧位微创PNCL手术可缩短肾结石患者治疗时间,提高穿刺及结石清除率。Objective:To analyze the effect of minimally invasive percutaneous nephrolith otripsy(PNCL)in the treatment of renal calculi in lateral recumbent position.Methods:From August 2018 to August 2020,50 patients with renal calculi were selected,they were randomly divided into the two groups with 25 cases in each group.All patients were treated with PNCL operation.The control group took the prone position.The experimental group took the lateral position.The operation index,puncture success rate,first-stage stone clearance rate and the incidence of complications were compared between the two groups.Results:The time of operation,the time of establishing passage and the time of getting out of bed in the experimental group were shorter than those in the control group,the intraoperative blood loss and the total time of lithotripsy in the experimental group were lower than those in the control group,and the success rate of puncture and the stone clearance rate in the first stage in the experimental group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).There was no significant difference in operation time and complication rate between the two groups(P>0.05).Conclusion:Minimally invasive PNCL operation in lateral recumbent position can shorten the treatment time of patients with renal calculi and improve the puncture and stone clearance rate.
