药物洗脱支架治疗左乳内动脉桥血管病变的单中心回顾性分析

摘　　要：目的:回顾性分析药物洗脱支架(DES)治疗左乳内动脉(LIMA)桥血管病变的临床情况,观察手术的安全性和近期疗效。方法:收集LIMA桥血管病变患者的临床资料和1年随访期间的主要心脏不良事件。结果:31例患者纳入研究,30例成功完成PCI治疗。与术前相比,术后1个月患者心绞痛CCS分级显著改善、左心室射血分数增加。所有患者完成1年临床随访,1例患者术后5个月心绞痛再发,冠状动脉造影显示右冠脉远端严重狭窄,支架植入后症状消失;22例完成造影随访,1例支架内再狭窄,其余患者无心绞痛发作、无心血管事件发生。结论:DES植入治疗LIMA桥血管病变可能是相对安全有效的选择。Objective:To retrospectively analyze the safety and short-term efficacy of drug eluting stent(DES)implanting in left internal mammary artery(LIMA)graft lesions.Methods:Clinical data of patients with LIMA undergoing DES implantation were selected.Major adverse cardiac events(MACEs)at admission and during one year follow-up were collected.Results:A total of 31 patients with LIMA undergoing PCI were enrolled in this study.All lesions were located in distal anastomosis.One case died during the procedure because of LIMA dissection and 30 patients successfully received DES implantation.Compared with pre-operation,the CCS grade was significantly improved and left ventricular ejection fraction was increased one month after the stenting.All patients completed a one-year follow-up.Recurrent angina pectoris was noted in one patient 5 months after PCI.Coronary angiography revealed severe stenosis in the distal end of right coronary,but angina pectoris relieved after DES implanting.22 patients received coronary angiography follow-up,with occurrence of target lesion restenosis in one patient and no MACE and angina pectoris occurring in the other patients.Conclusion:DES implantation may be a relatively safe and efficient choice for the treatment of LIMA.
