摘 要:目的旨在探讨炎症因子activin(Act)A与其结合蛋白Follistatin(FS)在急性心肌梗死(AMI)与慢性心力衰竭(CHF)患者血清中的水平变化及其来源与意义。方法应用ELISA法检测AMI组、CHF组及健康对照组血清Act A与FS水平,应用实时定量RT-PCR检测各组患者外周血单个核细胞Act A与FS mRNA表达,应用化学法Griess试剂测定各组患者外周血一氧化氮。结果AMI组血清activin A水平明显高于CHF组,CHF组显著高于健康对照组;AMI组血清FS水平升高,CHF组与健康对照组比较血清FS水平无明显变化;Act/FS比值:AMI组>CHF组>健康对照组。外周血单个核细胞Act A与FS mRNA表达水平在AMI组、CHF组及健康对照三组间无明显差异。AMI组及CHF组血清NO水平均较健康对照组明显升高,AMI组较CHF组升高更显著,其变化同血清Act A变化一致。结论AMI与CHF患者存在Act A-FS系统表达失衡,以Act A表达升高为主;高表达的Act A并非来源于单个核细胞,其可能通过促进分泌炎症介质NO,进一步参与AMI与CHF的病理过程。Objective To investigate the changes of inflammatory factor activin(Act)A and its binding protein Follistatin(FS)in serum of patients with acute myocardial infarction(AMI)and chronic heart failure(CHF),and its source and significance.Methods Serum levels of Act A and FS in AMI group,CHF group and healthy control group were detected by ELISA.The expressions of Act A mRNA and FS mRNA in peripheral blood mononuclear cells of patients in each group were detected by real-time quantitative RT-PCR.Nitric oxide(NO)in peripheral blood of patients in each group was determined by chemical method with Griess reagent.Results Serum Act A level in AMI group was significantly higher than that in CHF group,and CHF group was significantly higher than that in healthy control group.Serum FS level increased in AMI group,but there was no significant difference between CHF group and healthy control group.The Act/FS ratio was as follows:AMI group was higher than CHF group,and CHF group was higher than healthy control group.The mRNA expression levels of Act A and FS in peripheral blood mononuclear cells were not significantly different among AMI group,CHF group and healthy control group.Serum NO level in AMI group and CHF group was significantly higher than that in healthy control group,and the increase in AMI group was more significant than that in CHF group,which was consistent with the change of serum Act A.Conclusion The expression of Act A-FS system was unbalanced in patients with AMI and CHF,mainly manifested as the increased expression of Act A.The high expression of Act A does not originate from peripheral blood mononuclear cells,and it may be further involved in the pathological process of AMI and CHF by promoting the secretion of NO.
