针灸治疗腕管综合征临床选穴规律分析研究

摘　　要：目的:通过现代文献的分析,研究整理针灸治疗腕管综合征的临床选穴规律。方法:通过从中国知网(CNKI)、万方数据知识服务平台和维普数据库中检索出1986—2017年针灸治疗腕管综合征相关的临床文献,建立针灸治疗腕管综合征的处方数据库。借助“中医传承辅助平台”,分析归纳针灸治疗腕管综合征在临床上的选穴规律。结果:通过分析发现,现代医家治疗腕管综合征选取的腧穴主要为大陵、合谷、内关、外关、阳溪和鱼际;腧穴所属经脉主要为手三阴经中的手厥阴心包经和手三阳经中的手阳明大肠经、手少阳三焦经;所选腧穴的所在部位主要为上肢部;五输穴是主要选取的特定穴,其次是原穴和络穴,再次是八脉交会穴。结论:针灸治疗腕管综合征对于早期、轻症效果明显,临床选穴以局部取穴为主,循经取穴为辅,重用特定穴,并且常在病变部位选穴。Objective:To study the rules of acupoints selection in terms of acupuncture and moxibustion in treating carpal tunnel syndrome(CTS)through analysis of modern literature.Methods:The clinical literature related to acupuncture and moxibustion in treating CTS were retrieved from the databases of CNKI,Wanfang and VIP from 1986 to 2017,and the prescription database of acupuncture and moxibustion for CTS was established.With the help of the TCMISS,the rules of acupoints selection in terms of acupuncture and moxibustion treating CTS were analyzed and summarized.Results:It was found that the main acupoints selected by modern doctors for CTS were Daling(PC7),Hegu(LI4),Neiguan(PC6),Waiguan(SJ5),Yangxi(LI5)and Yuji(LU10).The location of the selected acupoints are mainly from Hand-Jueyin Pericardium Meridian,Hand-Yangming Large Intestine Meridian and Hand-Shaoyang Sanjiao Meridian.The selected acupoints were mainly located in the upper limbs.Five-Shu points were the specific acupoints,followed by Yuan-source acupoints,collateral points and eight confluence point.Conclusion:Acupuncture and moxibustion has a significant effect in the treatment of early and mild cases of CTS.The acupoints selected are mainly local acupoints,supplemented by acupoints along the meridian.The specific acupoints being heavily used and local points also being often used is the rule of the acupoints selection in the treatment of the disease.
