摘 要:目的探讨颅脑损伤后血清神经元特异性烯醇化酶(neuron-specific enolase, NSE)与S100B蛋白水平的动态性变化规律及临床意义。方法选取武警浙江省总队医院急诊科2018年1月-2019年12月接收的105例颅脑损伤患者为研究对象,按格拉斯哥评分分为轻度组(30例)、中度组(45例)和重度组(30例),另选同期健康体检者30例作为对照组。动态监测并统计分析患者颅脑损伤后血清NSE与S100B蛋白数据。结果重度组、中度组、轻度组、对照组4组血清NSE与S100B蛋白水平分别为(34.51±5.39)μg/L、(19.31±2.72)μg/L、(13.68±3.02)μg/L、(6.11±2.35)μg/L与(2.29±0.28)μg/L、(1.47±0.15)μg/L、(0.81±0.18)μg/L、(0.32±0.09)μg/L,F=357.010,653.126,均P<0.001;各组同时间点血清NSE与S100B蛋白比较差异有统计学意义(均P<0.05);血清NSE与S100B组间及组内不同时点两两比较差异均有统计学意义(均P<0.05)。结论血清NSE与S100B蛋白水平能够动态性地反映急性颅脑损伤的病情进展,对病情分级修正、制定治疗方案及评估预后均有重要的临床指导意义。Objective To explore the dynamic changes and clinical significance of serum NSE and S100B protein levels after craniocerebral injury.Methods A total of 105 patients with craniocerebral injury received by the Emergency De-partment of Zhejiang General Hospital of Armed Police from January 2018 to December 2019 were selected as the study objects.According to Glasgow's score,they were divided into mild group(n=30),moderate group(n=45)and severe group(n=30).Another 30 healthy people in the same period were selected as the control group.Dynamic monitoring and statistical analysis of serum NSE and S100B protein data after head injury were conducted.Results The serum NSE and S100B protein levels among the four groups were(34.51±5.39)μg/L,(19.31±2.72)μg/L,(13.68±3.02)μg/L,(6.11±2.35)μg/L and(2.29±0.28)μg/L,(1.47±0.15)μg/L,(0.81±0.18)μg/L,(0.32±0.09)μg/L(F=357.010,653.126,all P<0.001)respectively.For each group at the same time,the difference in serum NSE and S100B protein was statistically significant(all P<0.05).The differences in serum NSE and S100B between groups within the group and at different time points were statistically significant(all P<0.05).Conclusion Serum NSE and S100B protein levels can dynamically reflect the progress of acute craniocerebral injury a which have important clinical guiding significance for the revision of the disease disease classification,formulation of treatment plans assessment of prognosis.
