摘　　要：目的:探究自噬介导的急性ST段抬高型心肌梗死(STEMI)心肌缺血再灌注损伤中蛋白酶激活受体-2(PAR-2)、B细胞淋巴瘤因子-2(Bcl-2)、人抑癌基因Beclin1的表达及其相关性。方法:选取2017年1月至2018年7月诊断为STEMI患者50例为试验组,有胸痛症状但冠状动脉造影检查正常者50例为对照组,检测PCI术前及术后0 h、4 h、8 h、12 h、24 h、48 h血清PAR-2、Bcl-2及Beclin1水平,并分析三者的相关性。结果:试验组PAR-2、Bcl-2、Beclin1水平在PCI术前和术后各时间点均高于对照组(P<0.01)。PAR-2水平与Bcl-2呈正相关(r=0.932,P=0.000),Bcl-2水平与Beclin1呈负相关(r=-0.925,P=0.000)。结论:在STEMI心肌缺血-再灌注期,PAR-2、Bcl-2、Beclin1表达均升高,且三者间存在相关性,在减轻心肌缺血-再灌注损伤中发挥重要作用。Objective:To investigate the expression and correlations of PAR-2,Bcl-2 and Beclin1 in autophagy-mediated myocardial ischemia-reperfusion injury in patients with acute ST-segment elevation myocardial infarction(STEMI).Methods:50 patients with STEMI diagnosed from January 2017 to July 2018 were selected as the experimental group,and 50 patients with chest pain but normal coronary angiography were selected as the control group.The serum levels of PAR-2,Bcl-2 and Beclin1 were detected before PCI and 0 h,4 h,8 h,12 h,24 h,48 h after PCI,and the correlations among them were analyzed.Results:The levels of PAR-2,Bcl-2 and Beclin1 in the experimental group were higher than those in the control group before PCI and at different time points after PCI(P<0.01).PAR-2 level was positively correlated with Bcl-2(r=0.932,P=0.000),and Bcl-2 level was negatively correlated with Beclin1(r=-0.925,P=0.000).Conclusion:The expression of PAR-2,Bcl-2 and Beclin1 increases and autophagy is enhanced during myocardial ischemia-reperfusion in STEMI patients,and there are correlations among them,which plays an important role in alleviating myocardial ischemia-reperfusion injury.
