草酸钙结石患者24 h尿液代谢异常与代谢综合征的相关性

摘　　要：目的探讨草酸钙结石患者24 h尿代谢异常与代谢综合征(MS)的相关性。方法回顾性分析2018年11月至2019年11月于我院行手术治疗且结石成分分析为草酸钙的患者的临床资料,共计280例。分为MS组(74例)和非MS组(206例),分析两组患者24 h尿代谢异常数据的差异及相关影响因素。结果MS组低枸橼酸尿46例(62.2%),低于非MS组的153例(74.3%),差异具有统计学意义(P<0.05)。两组患者的高钠尿、高钙尿、高磷尿、低镁尿、高草酸尿、高尿酸尿患病率比较未见统计学差异(P>0.05)。进一步对低枸橼酸尿行多因素Logistic回归分析显示,校正性别后,高血糖的OR值为0.502(0.254~0.991)(P=0.047)。结论MS与草酸钙结石患者的高钠尿、高钙尿、高磷尿、低镁尿、高草酸尿、高尿酸尿无显著相关性,与低枸橼酸尿呈负相关关系,高血糖是低枸橼酸尿的独立保护因素。Objective To explore the relationship between 24-hour urine metabolic abnormalities and metabolic syndromes(MS)in patients with calcium oxalate stones.Methods The medical records of 280 adult patients whose stone composition analysis showed calcium oxalate were retrospectively analyzed.The patients were divided into the MS group(n=74)and non-MS group(n=206).The differences in 24-hour urine metabolic abnormalities and related influencing factors were analyzed between the two groups.Results In the MS group,46 cases had hypocitraturia,while 153 cases in the non-MS group had hypocitraturia(P<0.05).There were no significant differences in the prevalence of high urine sodium,hypercalciuria,hyperphosphaturia,hpomagnesuria,hyperoxaluria and hyperuricosuria between the two groups(P>0.05).Logistic regression analysis showed that,after adjustment of gender,the OR of hyperglycemia was 0.502(0.254-0.991)(P=0.047).Conclusion MS is not correlated with high urine sodium,hypercalciuria,hyperphosphaturia,hypomagnesuria,hyperoxaluria and hyperuricosuria,but negatively correlated with hypocitraturia.Hyperglycemia is an independent protective factor of hypocitraturia.
