输尿管软镜联合钬激光处理移植肾尿路结石7例疗效分析

摘　　要：目的探讨输尿管软镜治疗移植肾尿路结石的安全性及有效性。方法回顾性分析本院2014—2019年500例肾移植中发生的7例尿路结石的临床资料:其中移植肾结石3例、移植肾输尿管支架全程结石1例、输尿管结石3例。5例通过输尿管软镜取石,1例改为经皮肾镜联合输尿管软镜取石,1例改为经皮肾取石。结果7例移植肾尿路结石中,结石平均大小为11.5(6~17)mm,平均手术时间55(18~120)min;6例一次取净结石,1例输尿管支架全程结石2次取净结石,术后均未出现严重并发症和死亡。结论输尿管软镜治疗移植肾尿路结石安全、有效、微创。Objective To evaluate the safety and efficacy of flexible ureteroscope(F-URS)and holmium laser lithotripsy in the treatment of allograft kidney lithiasis.Methods The clinical data of 500 renal transplantations performed at our center during 2014 and 2019 were retrospectively analyzed.Allograft lithiasis was observed in 7 cases,including 3 cases of renal calculi,1 case of calculi in the ureteral stent and 3 cases of ureteral calculi.F-URS plus holmium laser lithotripsy was performed in 5 patients,percutaneous nephrolithotomy(PCNL)plus F-URS in 1 case,and PCNL in 1 case due to F-URS failure.Results The mean stone size was 11.5(6-17)mm,and mean operation time 55(18-120)minutes.Of all 7 patients,6 were stone-free after one F-URS procedure,and the patient with stones in the ureteral stent received a second-stage procedure.No severe complications or mortality occurred.Conclusion F-URS is a safe,effective and minimally invasive treatment modality for small-and medium-sized stones in allograft kidney lithiasis.
