腹腔镜下盆底抬高及直肠悬吊治疗出口梗阻型便秘的临床效果

摘　　要：目的研究出口梗阻型便秘患者采用腹腔镜下盆底抬高及直肠悬吊治疗的效果。方法随机抽取2013年1月至2019年12月入院接受诊治的出口梗阻型便秘患者56例,所有患者均使用腹腔镜下盆底抬高及直肠悬吊方法治疗,对比患者治疗前后粪便造影征象以及患者治疗前后的症状评分和排粪造影结果。结果治疗后骶直分离、直肠黏膜内套、直肠折曲、直肠前突患者数量明显少于治疗前,差异存在统计学意义(P<0.05),治疗前后盆底疝、会阴下降患者数量比较,差异无统计学意义(P>0.05)。治疗后患者便秘得分、伴随症状得分均低于治疗前,差异存在统计学意义(P<0.05);治疗后患者的乙耻距、肛上距、骶直间距指标值在下降,差异存在统计学意义(P<0.05)。结论腹腔镜下盆底抬高和直肠悬吊治疗出口梗阻型便秘疾病效果显著,可改善患者的症状,降低疾病对患者的影响。Objective To study the effect of laparoscopic pelvic floor elevation and rectal suspension in patients with outlet obstruction constipation.Methods 56 patients with outlet obstructive constipation were randomly selected from January 2013 to December 2019.All patients were treated with laparoscopic pelvic floor elevation and rectal suspension,the results of defecography before and after treatment were compared with the symptom scores and defecography results.Results After treatment,the number of patients with sacral-rectal dissection,rectal mucosa intussusception,rectal tortuosity and rectocele was significantly less than that before treatment,there was significant difference(P<0.05).There was no significant difference in the number of patients with pelvic floor hernia and perineum drop before and after treatment(P>0.05).After treatment,the scores of constipation and the scores of concomitant symptoms were lower than those before treatment(P<0.05),and the scores of second pubic distance,superior anal distance and sacral straight distance were lower than those before treatment(P<0.05).Conclusion Laparoscopic pelvic floor elevation and rectal suspension are effective in the treatment of outlet obstructive constipation,which can improve the symptoms and reduce the impact of the disease on patients.
