中药内服联合外用治疗哺乳期急性乳腺炎的临床疗效观察

摘　　要：目的观察消痈通乳汤联合芙蓉散治疗哺乳期急性乳腺炎的临床疗效。方法选取2018年12月至2019年12月在辽宁中医药大学附属第二医院外科门诊就诊的哺乳期急性乳腺炎患者90例,随机分为治疗组和对照组,每组45例。治疗组予以消痈通乳汤口服,日1剂,分2次口服,同时芙蓉散外敷。对照组予以青霉素注射液640万U,日2次静点,疗程共7 d。观察两组患者治疗前后白细胞计数(WBC)、中性粒细胞百分比(NE%)、C反应蛋白(CRP);治疗前后对乳房胀痛、红肿面积、乳汁分泌情况、肿块大小进行评分,判定临床疗效。结果治疗后两组患者的WBC、NE%、CRP均下降,两组间差异无统计学意义(P>0.05);治疗后两组患者乳房胀痛、红肿面积、乳汁分泌情况、肿块大小评分较治疗前下降,各项评分治疗组均低于对照组,两组间比较差异有统计学意义(P<0.05);治疗后治疗组有效率95.5%,对照组有效率82.2%,两组间比较差异有统计学意义(P<0.05)。结论消痈通乳汤联合芙蓉散治疗哺乳期急性乳腺炎效果显著。Objective To observe the clinical effect of Xiaoyong Tongru Decoction combined with Furong Powder in the treatment of lactation acute mastitis.Methods From December 2018 to December 2019,90 patients with acute mastitis during lactation were treated in the Surgical Outpatient Department of the Second Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,who were randomly divided into treatment group and control group,with 45 patients in each group.Patients in treatment group were given one dose of Xiaoyong Tongru Decoction orally,twice a day,combined with Furong Powder for external application.Patients in control group were given 6.4 million of penicillin injection U twice a day for 7 days.White blood cell count(WBC),neutrophil percentage(NE%)and C-reactive protein(CRP)were observed before and after treatment.Breast distension pain,redness and swelling area,breast milk secretion and mass size were scored before and after treatment to determine the clinical efficacy.Results After treatment,WBC,NE%and CRP of the two groups were decreased,and there was no significant difference between the two groups(P>0.05).After treatment,the scores of breast swelling pain,swelling area,breast milk secretion and mass size of the two groups were decreased compared with those before treatment.All the scores in treatment group were lower than those in the control group(P<0.05).The effective rate of the treatment group was higher than that of the control group(95.5%vs.82.2%,P<0.05).Conclusion Xiaoyong Tongru Decoction combined with Furong Powder has a significant effect on acute mastitis in lactation period.
