摘 要:目的探讨血栓调节蛋白(TM)启动子区-33GA基因多态性与良性阵发性位置性眩晕(BPPV)发病的相关性。方法选取2017年12月~2019年6月华中科技大学同济医学院附属荆州医院收治的BPPV患者60例作为BPPV组,另选取同期体检健康者60例作为对照组。运用聚合酶链式反应-限制性片段长度多态性方法(PCR-RELP)检测所有研究对象TM-33G/A基因多态性。通过多元logistic回归分析BPPV的危险因素。结果 BPPV组TM-33G/A位点G/A点突变率显著高于对照组(P<0.05);GA、AA型BPPV患者收缩压、舒张压、低密度脂蛋白显著高于GG型患者(P<0.05),GA型BPPV患者收缩压、舒张压、低密度脂蛋白显著高于AA型患者(P<0.05);高血压、低密度脂蛋白、TM-33G/A GA型、AA型是BPPV发病的危险因素(P<0.05)。结论 TM-33G/A位点突变与BPPV发生密切相关。OBJECTIVE To investigate the correlation between thrombomodulin(TM) promoter-33GA gene polymorphism and benign paroxysmal positional vertigo(BPPV).METHODS Sixty patients with BPPV admitted to Affiliated Jingzhou Hospital of Tongji Medical College of Huazhong Science and Technology University from December 2017 to June 2019 were selected as BPPV group,and 60 healthy persons matched by age and sex in the same period were selected as control group.Polymerase chain reaction-restriction fragment length polymorphism(PCR-RELP) was used to detect TM-33G/A gene polymorphism in all subjects.The risk factors of BPPV were analyzed by multiple logistic regression.RESULTS The G/A mutation rate of TM-33G/A site in BPPV group was significantly higher than that in control group(P<0.05);systolic blood pressure,diastolic blood pressure and low density lipoprotein in patients with GA and AA type BPPV were significantly higher than those in patients with GG type(P<0.05),while systolic blood pressure,diastolic blood pressure and low density lipoprotein in patients with GA type BPPV were significantly higher than those in patients with AA type(P<0.05);bivariate logistic regression analysis showed that hypertension,LDL,TM-33G/A GA and AA types were risk factors for BPPV(P<0.05).CONCLUSIONThe mutation of TM-33G/A locus is closely related to the occurrence of BPPV.
