结合雌激素乳膏联合米索前列醇应用于绝经后取环的价值研究

摘　　要：目的分析结合雌激素乳膏联合米索前列醇应用于绝经后取环的价值。方法选取2017年1月~2020年1月无锡市锡山人民医院妇产科收治的80例绝经后取环患者作为研究对象,按照随机数字表法分为对照组(n=40)与观察组(n=40),对照组术前1 h给予米索前列醇400μg肛塞,观察组每天给予1 g结合雌激素乳膏阴道用药1周,且术前1 h给予米索前列醇400μg肛塞。比较两组的术中扩张过程视觉模拟评分、术中出血量、取环时间、宫颈扩张总有效率、取环成功率以及不良反应。结果观察组的术中扩张过程视觉模拟评分低于对照组,术中出血量少于对照组,取环时间短于对照组,差异均有统计学意义(P<0.05)。宫颈扩张总有效率、取环成功率均高于对照组,差异均有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论结合雌激素乳膏联合米索前列醇应用于绝经后取环的价值高,结合雌激素乳膏阴道用药简单方便,雌激素用量小,可有效扩张宫颈,缩短取环时间,减少术中出血和提高取环成功率,减轻患者痛苦,值得推广。Objective To analyze the value of Conjugated Estrogen Cream combined with Misoprostol in ring removal after menopause.Methods A total of 80 postmenopausal patients who were treated in Xishan People′s Hospital from January 2017 to January 2020 were divided into control group(n=40)and observation group(n=40)according to random number table method.The control group were given Misoprostol 400μg anal plug 1 h before operation,and the observation group was given 1 g of Conjugated Estregen Cream for vaginal medication for 1 week every day,and Misoprostol 400μg anal plug 1 hour before operation.The visual analogue scores of the intraoperative dilation process,intraoperative blood loss,time of ring removal,total effective rate of cervical dilation,success rate of ring removal,and adverse reactions were compared between the two groups.Results The visual simulation score of intraoperative dilation of the observation group was lower than that of the control group,the amount of intraoperative blood loss was less than that of the control group,and the ring removal time was shorter than that of the control group,the differences were statistically significant(P<0.05).The total effective rate of cervical dilation and the success rate of ring removal were higher than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Conjugated Estrogen Cream combined with Misoprostol has a high value in the application of postmenopausal ring removal.Conjugated Estrogen Cream is simple and convenient for vaginal administration,and the amount of Estrogen is small.It can effectively dilate the cervix,shorten the time of ring removal,reduce bleeding and improve the success rate of ring removal,and alleviate the pain of patients,which is worthy of promotion.
