腹膜透析患者腹膜纤维化与腹膜透析液中MMP-2、TIMP-1及IL-6水平的相关性研究

摘　　要：目的探讨腹膜透析患者腹膜纤维化(PF)与腹膜透析液中基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶组织抑制因子-1(TIMP-1)及白细胞介素-6(IL-6)水平的相关性。方法选取2018年1月—2019年12月行维持性腹膜透析致PF的38例患者作为观察组,并纳入同期行维持性腹膜透析未发生PF的50例患者作为对照组。比较2组PF相关指标[转化生长因子-β1(TGF-β1)、纤维粘连蛋白(FN)、结缔组织生长因子(CTGF)]及MMP-2、TIMP-1、IL-6水平,并分析MMP-2、TIMP-1、IL-6与PF相关指标的相关性。结果观察组腹膜透析液中TGF-β1、FN、CTGF水平均高于对照组(P<0.01)。观察组腹膜透析液中MMP-2水平低于对照组,TIMP-1、IL-6水平高于对照组,差异有统计学意义(P<0.01)。Pearson相关分析显示:MMP-2与TGF-β1、FN、CTGF均呈负相关(P<0.05或P<0.01),TIMP-1、IL-6与TGF-β1、FN、CTGF均呈正相关(P<0.01)。结论监测腹膜透析致PF患者腹膜透析液PF相关指标有利于了解患者PF病情进展程度,MMP-2/TIMP-1比例失衡及腹膜炎症反应可能是PF发生进展的原因。Objective To investigate correlations between peritoneal fibrosis(PF)with levels of matrix metalloproteinase-2(MMP-2),tissue inhibitor of matrix metalloproteinase-1(TIMP-1)and interleukin-6(IL-6)in peritoneal dialysate of patients undergoing peritoneal dialysis(PD).Methods A total of 38 PF patients caused by maintenance PD during January 2018 and December 2019 were selected as observation group,while other 50 patients without PF undergoing maintenance PD at the same period were included as control group.Levels of PF-related indexes[transforming growth factor-β1(TGF-β1),fibronectin(FN)and connective tissue growth factor(CTGF)]and MMP-2,TIMP-1 and IL-6 were compared,and correlations between MMP-2,TIMP-1 and IL-6 with PF-related indexes were also analyzed in two groups.Results In observation group,levels of TGF-β1,FN and CTGF in peritoneal dialysate were significantly higher than those in control group(P<0.01);in peritoneal dialysate,MMP-2 level was significantly lower,while TIMP-1 and IL-6 levels were significantly higher than those in control group(P<0.01).Pearson correlation analysis showed that MMP-2 was negatively correlated with TGF-β1,FN and CTGF(P<0.05 or P<0.01),while TIMP-1 and IL-6 were positively correlated with TGF-β1,FN and CTGF(P<0.01).Conclusion Monitoring PF-related indicators of peritoneal dialysate in PF patients caused by PD with is helpful to learn progression of PF.The imbalance of MMP-2/TIMP-1 ratio and peritoneal inflammatory reaction may contribute to pathogenesy and progression of PF.
