摘　　要：目的观察芪参益气滴丸对非阻塞性冠状动脉缺血疾病(INOCA)患者冠状动脉微循环功能障碍(CMD)的疗效与安全性。方法对2019年1月—2020年11月期间主因胸闷、胸痛症状就诊于本院心内科经冠状动脉造影或冠脉CTA诊断为INOCA的患者进行心肌声学造影(MCE)检查,通过定量测定冠脉血流储备(CFR)评价其冠状动脉微血管功能,将60例CFR<2即冠脉微循环功能障碍者纳入研究,按照随机数字表法分为试验组(n=30)和对照组(n=30)。试验组给予芪参益气滴丸联合常规西药治疗,对照组给予安慰剂联合常规西药治疗,比较两组患者治疗前和治疗3个月后的临床症状、微循环功能变化及不良反应发生情况。结果试验组患者生活质量及冠脉微循环功能改善率明显高于对照组(P<0.05);不良反应发生情况比较差异无统计学意义(P>0.05)。结论芪参益气滴丸对INOCA患者的冠脉微循环功能障碍有改善作用且安全性良好,为中药联合常规西药改善INOCA患者的冠脉微循环功能提供了临床依据。Objective To determine the efficacy and safety of Qishen Yiqi dropping pills on coronary microvascular dysfunction in patients with ischaemic non-obstructive coronary arteries(INOCA).Methods Patients diagnosed with INOCA by coronary angiography or coronary CTA in the Department of Cardiology of the First Hospital of Jilin University due to chest tiredness and chest pain from January 2019 to November 2020 were examined by myocardial contrast echocardiography.Coronary microvascular function was evaluated by quantitative determination of Coronary blood flow reserve(CFR),and 60 patients with CFR<2,namely with coronary microcirculation dysfunction,were included and divided into an experimental group(n=30)and a control group(n=30)randomly.The experimental group was given Qishen Yiqi dropping pills in addition to conventional western medicine treatment,while the control group was given placebo and conventional western medicine treatment.The quality of life,changes in microcirculation function and occurrence of adverse reactions were compared between the two groups before the treatment and 3 months after the treatment.Results The quality of life and the improvement in coronary microcirculation function in the experimental group were significantly higher than those in the control group,with significant difference(P<0.05).No significant difference was found in the incidence of adverse reactions.Conclusion Qishen Yiqi dropping pills can improve the coronary microcirculation dysfunction in patients with INOCA safely,provideing a clinical basis for TCM combined with conventional western medicine treatment to improve the coronary microcirculation function in INOCA patients.
