四子散腹部穴位热熨对人工髋关节置换术后便秘的预防效果及对关节功能的影响

摘　　要：目的:观察四子散腹部穴位热熨联合早期下床活动对人工髋关节置换术后便秘的预防效果及对关节功能的影响。方法:回顾性分析我院2018年1月~2020年12月在本院择期行人工髋关节置换术的患者174例,按照护理方法不同,分为观察组(n=87)和对照组(n=87)。对照组术后给予常规对症治疗及护理,观察组在对照组的基础上行四子散腹部穴位热熨联合早期下床活动干预,比较两组患者术后便秘症状评分,记录患者术后首次自主排便时间、每分钟肠鸣音次数、术后1个月并发症发生情况和Harris髋关节功能评分。结果:观察组患者的首次自主排便时间为(30.25±8.64)h,短于对照组的(38.47±9.23)h(P<0.05);术后1d、2d和3d,观察组患者的平均每分钟肠鸣音次数均高于对照组(P<0.05);术后1个月,观察组患者的并发症发生率为14.94%,低于对照组的29.89%(P<0.05);干预后,观察组的便秘症状评分低于对照组(P<0.05);干预后,观察组患者的Harris髋关节评分总分、日常功能及活动度各项评分低于对照组(P<0.05)。结论:四子散腹部穴位热熨联合早期下床活动在预防人工髋关节置换术后便秘、促进关节功能恢复方面,疗效确切,可明显改善患者便秘症状,降低术后并发症发生率,加速髋关节功能恢复。Objective:To explore the preventive effect of Sizisan Powder hot compress at abdominal acupoints combined with early off-bed activity on constipation and its influences on joint function after artificial hip replacement.Methods:A retrospective analysis of 174 patients with hip arthroplasty in our hospital from January 2018 to December 2020 in our hospital was conducted.According to different nursing methods,they were divided into an observation group(n=87)and a controlled group(n=87).The controlled group was given conventional symptomatic treatment and nursing,while the observation group was given routine symptomatic treatment and nursing,and the observation group was intervened by Sizisan Powder abdominal acupoint hot ironing combined with early ambulation intervention on the basis of the controlled group.The postoperative constipation symptom scores of the two groups were compared,and the first spontaneous defecation time,the frequency of bowel sounds per minute,the incidence of complications one month after operation and Harris hip joint function score were recorded.Results:The first autonomous defecation time in the observation group was shorter than that in the controlled group[(30.25±8.64)h vs(38.47±9.23)h](P<0.05).At 1 d,2 d and 3 d after surgery,times of bowel sounds per minute in the observation group were more than those in the controlled group(P<0.05).At 1 month after surgery,the incidence of complications in the observation group was lower than that in the controlled group(14.94%vs 29.89%)(P<0.05).After intervention,the constipation symptom score of the observation group was lower than that of the controlled group(P<0.05).After intervention,the total score of Harris hip,scores of daily function and activity in the observation group were lower than those in the controlled group(P<0.05).Conclusion:The curative effect of Sizisan Powder hot compress at abdominal acupoints combined with early off-bed activity is significant on preventing constipation and promoting joint function recovery after artificial
