邵氏针灸组穴治疗对糖尿病周围神经病变患者血清TNF-α、PGl2、IFN-γ水平的影响

摘　　要：目的:观察邵氏针灸组穴治疗对糖尿病周围神经病变(DPN)患者血清肿瘤坏死因子-α(TNF-α)、前列环素(PGl2)、干扰素-γ(IFN-γ)水平的影响。方法:选择2018年7月~2020年7月于本院就诊的148例DPN患者,按随机数表法分为观察组74例和对照组74例。对照组予常规西医药物治疗,观察组在其基础上应用邵氏针灸组穴治疗。观察两组临床疗效、神经症状评分、神经电生理检查结果、血糖水平变化、血清TNF-α、PGl2、IFN-γ水平及不良反应。结果:治疗后,观察组总有效率为95.95%,高于对照组总有效率86.49%(P<0.05);观察组神经症状评分低于对照组(P<0.05);观察组腓总神经、正中神经传导速度均高于对照组(P<0.05);观察组空腹血糖、餐后2h血糖、糖化血红蛋白水平均低于对照组(P<0.05);观察组TNF-α、IFN-γ水平均低于对照组,PGl2水平高于对照组(P<0.05);两组治疗期间均未出现不良反应。结论:邵氏针灸组穴治疗对DPN患者有较好疗效,不仅能够促进神经症状、神经传导速度恢复,对血糖调控同样具有积极影响,抗炎效果显著,同时有效减少细胞组织损伤,安全性较好。Objective:To study the effect of SHAO’s acupuncture group on levels of serum tumor necrosis factor-α(TNF-α),prostacyclin(PGl2)and interferon-γ(IFN-γ)for patients with diabetic peripheral neuropathy(DPN).Methods:A total of 148 DPN patients who were treated in the hospital from July 2018 to July 2020 were enrolled and divided into an observation group(74 cases)and a controlled group(74 cases)by the random number table method.The controlled group was treated with routine western medicine,while the observation group was treated with SHAO’s acupuncture group on the basis of the controlled group.The clinical curative effect,scores of neurological symptoms,examination results of nerve electrophysiology,changes of blood glucose level,levels of serum TNF-α,PG12 and IFN-γ,and adverse reactions in both groups were observed.Results:After treatment,the total response rate of the observation group was higher than that of the controlled group(95.95%vs 86.49%)(P<0.05).The scores of neurological symptoms in the observation group were lower than those in the controlled group(P<0.05),conduction velocity of common peroneal nerve and median nerve was faster than that in the controlled group(P<0.05),levels of fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin were lower than those in control group(P<0.05),levels of TNF-αand IFN-γwere lower than those in the controlled group,and level of PG12 was higher than that in the controlled group(P<0.05).There were no adverse reactions during treatment in either group.Conclusion:The application effect of SHAO’s acupuncture group is good in DPN patients.It can not only promote the recovery of neurological symptoms and nerve conduction velocity,with positive effects on blood glucose regulation and significant anti-inflammatory effect,but also can effectively reduce cells damage,with high safety.
