摘 要:目的:探讨呼出气一氧化氮(FeNO)水平与低出生体质量儿支气管肺发育不良(BPD)的关系。方法:采用回顾性研究方法,选取2013年8月至2018年2月湖北省黄石市爱康医院新生儿重症监护病房收治的低出生体质量儿94例,根据BPD发病情况分为BPD组和非BPD组,所有患儿均给予FeNO检测,同时收集患儿临床资料进行相关性分析。结果:94例低出生体质量儿中,BPD发病率为10.6%。BPD组的出生胎龄小于非BPD组,有创通气时间、需氧吸入时间均长于非BPD组,使用肺表面活性物质治疗、雾化吸入激素治疗的比例均高于非BPD组(P<0.05)。BPD组与非BPD组出院时的FeNO水平分别为(13.67±4.29)ppb、(8.09±3.10)ppb(P<0.05)。BPD的FeNO最佳阈值为11.55 ppb,诊断特异度为75.0%、灵敏度为72.7%。Logistic回归分析显示出生胎龄、FeNO水平是导致BPD发病的危险因素(P<0.05)。结论:FeNO参与了低出生体质量儿BPD的病理生理过程,FeNO检测具有无创、快速、重复性好等优点,可相对客观地评价生后早期肺功能,有效反映BPD发病状况。Objective:To probe into the correlation between fractional exhaled nitric oxide(FeNO)level and bronchopulmonary dysplasia(BPD)in low birth weight infants.Methods:Retrospective analysis was adopted,94 cases of low birth weight infants admitted to the neonatal intensive care unit of Hubei Huangshi Aikang Hospital from Aug.2013 to Feb.2018 were extracted to be divided into the BPD group and non-BPD group according to the incidence of BPD.All infants were given FeNO test,and clinical data were collected for correlation analysis.Results:Among the 94 cases of low birth weight infants,the incidence of BPD was 10.6%.The gestational age at birth in the BPD group was lower than that of the non-BPD group,the invasive ventilation time and aerobic inhalation time were longer than those of the non-BPD group.The proportions of pulmonary surfactant and aerosol inhalation of hormone treatment were higher than those of the non-BPD group(P<0.05).The FeNO level in the BPD group and the non-BPD group at discharge was respectively(13.67±4.29)ppb and(8.09±3.10)ppb(P<0.05).The optimal threshold of FeNO for BPD was 11.55 ppb,the diagnostic specificity was 75.0%,and the sensitivity was 72.7%.Logistic regression analysis showed that gestational age at birth and FeNO level were the risk factors for BPD(P<0.05).Conclusion:FeNO participates into the pathophysiological process of BPD in low birth weight infants.FeNO detection has the advantages of noninvasive,rapid and good reproducibility,which can objectively evaluate the pulmonary function in the early postnatal period and effectively reflect the incidence of BPD.
