多平面超声成像技术在产后肛门外括约肌损伤诊断中的价值

摘　　要：目的探讨利用多平面超声成像技术评价产后肛门外括约肌损伤的可行性和有效性。方法两位观察者利用多平面超声成像技术评估656例阴道分娩女性肛门外括约肌的完整性,分析肛门外括约肌损伤的声像图特征,并比较观察者间评价肛门外括约肌损伤的一致性。结果多平面超声成像技术诊断肛门外括约肌损伤8例,会阴侧切组与无会阴侧切组肛门外括约肌损伤率比较差异无统计学意义(P>0.05)。两位观察者评价肛门外括约肌损伤的一致性好(Kappa值=0.89)。结论多平面超声成像技术能够提供有价值的肛门括约肌解剖信息,对肛门外括约肌损伤的诊断具有较高的一致性,本研究结果支持利用多平面超声成像技术评价肛门外括约肌损伤的临床有效性。Objective To discuss the feasibility and effectiveness of multiplanar ultrasound imaging technique in the evaluation of postnatal external anal sphincter injury.Methods Two observers evaluated the integrity of external anal sphincter in 656 vaginal delivery women by using multiplanar ultrasound imaging technique,analyzed the sonographic characteristics of external anal sphincter injury,and compared the consistency of the evaluation of external anal sphincter injury between the observers.Results Eight cases of external anal sphincter injury were diagnosed by multiplanar ultrasound imaging technique.There was no significant difference in the rate of external anal sphincter injury between the group with episiotomy and without episiotomy(P>0.05).There was a good agreement between the two observers in the evaluation of external anal sphincter injury(Kappa=0.89).Conclusions Multiplanar ultrasound imaging technique can provide valuable anal sphincter anatomical information and has a high consistency in the diagnosis of external anal sphincter injury.This study support the clinical effectiveness of multiplanar ultrasound imaging technique in the evaluation of external anal sphincter injury.
