改良切开减压术联合外剥内扎术治疗环状混合痔的临床疗效

摘　　要：为探讨改良切开减压术联合外剥内扎术治疗环状混合痔的临床疗效,将2018年2月至2020年2月我科收治的110例环状混合痔患者随机分为观察组和对照组,各55例,观察组采用改良切开减压术联合外剥内扎术治疗,对照组采用常规外剥内扎术治疗。比较2组临床总体疗效,术后疼痛、水肿、出血程度,住院时间和创面愈合时间,术后6个月肛门狭窄、肛门失禁情况。结果显示,2组总有效率均为100%。2组患者住院时间、创面愈合时间、术后6个月肛门失禁情况比较差异均无统计学意义,P>0.05。观察组患者术后疼痛、水肿、出血程度评分均明显低于对照组,P<0.05。术后随访6个月,观察组患者肛门狭窄情况明显优于对照组,P<0.05。结果表明,改良切开减压术联合外剥内扎术治疗环状混合痔疗效优于常规外剥内扎术。This study was to investigate the effect of modified incision &decompression combined with external dissection &internal ligation in the treatment of circumferential mixed hemorrhoids(CMH),randomly divided 110 CMH patients treated in authors’ hospital(2018-02-2020-2)into observation group(55 cases,adopted above-mentioned combination surgery)and control group(55 cases,alone external dissection &internal ligation);then,compared both groups’ whole effect;postoperative degree of pain,of edema,and of bleeding;the time for hospitalization,and for wound-surface healing;as well as the status on anal stenosis,on anal incontinence 6 months after surgery.As results,both groups’ total effective rate was all up to 100%;in hospitalization time,in wound-surface healing time,and in anal incontinence 6 months after surgery there was no statistical difference between the two groups(P >0.05);in the score ratings on postoperative pain,edema and bleeding observation group was significantly lower than control group(P<0.05);the follow-up of 6 months after surgery in anal stenosis observation group was significantly superior to control group(P<0.05).Results show that combined surgery has more better effect than alone external dissection &internal ligation.
