摘 要:目的探讨D-乳酸和二胺氧化酶的变化对新生儿败血症患儿胃肠黏膜屏障功能损伤的临床意义。方法选取西安市儿童医院新生儿科2017年7月至2018年6月收治的新生儿败血症患儿97例为病例组,将其分为无胃肠道功能障碍组(27例)和胃肠道功能障碍组(70例),选取同期体检的30例健康新生儿为对照组。采取静脉血进行血清D-乳酸、二胺氧化酶活性的测定,并进行分析。结果病例组败血症患儿的D-乳酸水平和二胺氧化酶活性均显著高于对照组,但两组比较差异均无统计学意义(均P>0.05)。在病例组中,胃肠道功能障碍组与无胃肠道功能障碍组败血症患儿的D-乳酸水平和二胺氧化酶活性比较差异均有统计学意义(t值分别为6.770、6.127,均P<0.05)。结论对新生儿败血症患儿D-乳酸和二胺氧化酶的测定可以早期评判新生儿败血症患儿胃肠道黏膜屏障功能的受损情况,给予及时的相关治疗干预,减少多器官功能障碍综合征的发生。Objective To explore the clinical significance of changes in D-lactose and diamine oxidase for gastrointestinal mucosal barrier function in neonates with sepsis.Methods 97 newborns with sepsis admitted from July 2017 to June 2018 in the Department of Neonatology,Xi’an Children’s Hospital were selected as the case group,that were divided into gastrointestinal dysfunction group and non-gastrointestinal dysfunction group.And 30 healthy newborns who performed physical examination in the same period were selected as the control group.Venous blood was taken for the determination of the activity of serum D-lactic acid and diamine oxidase,and the activity of serum D-lactic acid and diamine oxidase were analyzed.Results The level of D-lactate and activity of diamine oxidase children with sepsis in the case group were significantly higher than those in the control group,but the differences between the two groups were not statistically significant(all P>0.05).In the case group,the difference in the level of D-lactate level and activity of diamine oxidase in children with sepsis between the gastrointestinal dysfunction group and non-gastrointestinal dysfunction group was statistically significant(t=6.770 and 6.127,respectively,both P<0.05)Conclusion The determination of D-lactic acid and diamine oxidase in neonates with sepsis can be used for early evaluation of the damage of gastrointestinal function,and providing relevant therapeutic intervention in time,and to reduce the occurrence of multiple organ dysfunction syndrome.
