摘 要:目的研究高血压患者血压水平与面部红外热像改变的关系,探索一种快捷、无接触评估高血压状态的方法。方法 2019年1-12月,纳入重庆医科大学附属第二医院心血管内科已确诊的住院高血压患者101例,根据即刻测量血压值,分为即刻血压未达标组[收缩压(165.6±17.8) mm Hg]61例和即刻血压达标组[收缩压(121.4±14.8)mm Hg]40例。使用红外相机采集所有受试者的面部热像,选取特定面部观察区(ROI)并获得热像温度参数,分析其与血压关系。结果与即刻血压达标组相比,即刻血压未达标组患者的面部ROI温度[(34.62±0.69)比(33.33±0.68)℃]较高(P<0.001);所有受试者面部ROI温度和实时收缩压呈正相关(r=0.714,P<0.01);受试者工作特征(ROC)曲线结果显示,高血压控制达标的最佳温度截点为33.95℃,曲线下面积(AUC)=0.921(P<0.01),敏感度86.9%,特异度82.5%。结论高血压患者即刻血压与面部ROI温度呈正相关,红外热像具有无接触监测高血压患者血压状态的潜能。Objective To study the relationship between blood pressure levels and the temperature reading obtained from facial infrared thermal imaging, and to explore a convenient non-contact method for assessing the blood pressure status. Methods Between January to December 2019, a total of 101 hypertensive patients were enrolled from the Department of Cardiology of the Second Affiliated Hospital of Chongqing Medical University. Based on real-time measurement of blood pressure, patients were divided into immediate blood pressure controlled group [61 patients, systolic blood pressure(SBP)(165.6±17.8) mm Hg] and immediate blood pressure uncontrolled group [40 patients, SBP(121.4±14.8) mm Hg]. Facial thermal images of all patients were collected by the infrared thermal camera and regions of interest(ROI) were set by infrared thermal imaging analysis software. The relationship between temperature parameter and blood pressure was analyzed. Results The mean cheek temperature of ROI was significantly higher in immediate blood pressure controlled group than immediate blood pressure uncontrolled group [(34.62±0.69) vs(33.33±0.68) ℃, P<0.001]. The mean temperature of ROI positively correlated with immediate blood pressure in all patients(r=0.714, P<0.01);The receiver operating characteristic(ROC) curve results showed that the optimal temperature cut-off point for hypertension control was 33.95 ℃, area under curve(AUC)=0.921(P<0.01). The sensitivity and specificity were 86.9% and 82.5% respectively. Conclusions The immediate blood pressure in hypertensive patients is positively correlated with facial temperature of ROI. Infrared thermography can potentially monitor the blood pressure status of hypertensive patients without direct contact.
