摘 要:目的探讨血液透析滤过(HDF)联合小剂量骨化三醇对肾性骨病(ROD)患者血清骨源性碱性磷酸酶(BAP)、甲状旁腺素(iPTH)水平及钙磷代谢的影响。方法选取解放军庐山康复疗养中心2018年2月—2020年2月收治的ROD患者87例,依据抽签法分为对照组43例和观察组44例。2组均予以小剂量骨化三醇治疗,在此基础上,对照组予以常规血液透析(HD)治疗,观察组予以HDF治疗。比较2组治疗前后血清BAP、iPTH水平,钙、磷水平,血肌酐(SCr)、血尿素氮(BUN)、白蛋白(ALB),并比较2组治疗期间不良反应发生情况。结果治疗前2组BAP、i PTH水平,钙、磷水平,SCr、BUN、ALB水平比较,差异无统计学意义(P>0.05);治疗后观察组BAP、i PTH水平低于对照组,钙水平高于对照组,磷水平低于对照组,SCr、BUN水平低于对照组,ALB水平高于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 ROD患者采用HDF联合小剂量骨化三醇治疗效果良好,可有效促进机体钙磷代谢,改善肾功能,且安全性较高。Objective To investigate the effect of hemodiafiltration(HDF)combined with low-dose calcitriol on serum levels of bone derived alkaline phosphatase(BAP),parathyroid hormone(iPTH)and calcium and phosphorus metabolism in patients with ROD.Methods A total of 87 cases of patients with rod were selected from February 2018 to February 2020 in People’s Liberation Army Lushan Rehabilitation Center,which were divided into control group(43 cases)and observation group(44 cases)by drawing lots.Both groups were treated with low-dose calcitriol.On this basis,the control group was treated with conventional hemodialysis(HD),and the observation group was treated with HDF.The levels of serum BAP,iPTH,calcium and phosphorus,serum creatinine(SCr),blood urea nitrogen(BUN)and albumin(ALB)before and after treatment were compared between the two groups,and the incidence of adverse reactions during treatment was compared between the two groups.Results Before treatment,there was no significant difference in the levels of BAP,iPTH,calcium,phosphorus,SCr,BUN and ALB between the two groups(P>0.05);after treatment,the levels of BAP and iPTH in the observation group were lower than those in the control group,the levels of calcium were higher than those in the control group,the levels of phosphorus were lower than those in the control group,the levels of SCr,BUN were lower than those in the control group,and the levels of Alb were higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion HDF combined with low-dose calcitriol can effectively promote calcium and phosphorus metabolism,improve renal function,and with high safety.
