摘　　要：目的:探讨头孢噻利联合阿米卡星治疗革兰阴性杆菌所致肺部感染与尿路感染的有效性与安全性。方法:采用回顾性研究方法,收集2019年我院采用头孢噻利联合阿米卡星或美罗培南治疗的革兰阴性杆菌肺部感染与尿路感染患者的病历资料,对比分析两种治疗方案的主要临床有效性参数[临床有效率、体温正常时间、临床肺部感染评分(CPIS)、尿白细胞计数(WBC)]、微生物学有效性参数(细菌清除率)以及药品不良反应发生率。结果:两组患者的临床有效率、细菌清除率,体温恢复正常时间、抗菌药物疗程,以及药品不良反应发生率等差异均无统计学意义(P>0.05)。治疗3,7 d后,两组肺部感染患者的CPIS评分和尿路感染患者的尿WBC水平及其下降值比较,差异均无统计学意义(P>0.05)。结论:头孢噻利联合阿米卡星治疗革兰阴性杆菌感染可能具有较好的有效性、安全性,可作为中重度感染患者的一种选择。Objective:To investigate the effectiveness and safety of cefoselis combined with amikacin for pulmonary infections or urinary tract infections induced by Gram-negative bacilli.Methods:A retrospective study on cefoselis combined with amikacin or meropenem used in patients with pulmonary or urinary tract infections induced by Gram-negative bacilli was performed.The research indicators included general data,main clinical effectiveness indicators(CPIS,WBC in urine,time to normal temperature,clinical effective rates),microbiology effectiveness(rate of bacteria clearance)and incidence of adverse drug reactions.Results:The levels of CPIS and WBC in urine on the first,third and seventh day were similar between the groups of cefoselis with amikacin or meropenem(P>0.05).The time to normal temperature,clinical effective rates and rates of bacteria clearance were similar between the groups of cefoselis with amikacin or meropenem(P>0.05).The incidences of adverse drug reactions were similar between the groups of cefoselis with amikacin or meropenem(P>0.05).Conclusion:The treatment of cefoselis combined with amikacin can acquire good effectiveness and safety for pulmonary infections and urinary tract infections induced by Gram-negative bacilli,which can be used as a choice for patients with moderate and severe infections.
