摘　　要：目的探讨外置钢板加封闭式负压引流(VAD)在治疗软组织严重损伤的胫骨开放粉碎性骨折中的应用价值。方法选取本院2018年5月至2020年5月收治的胫骨开放粉碎性骨折患者84例,采用电脑盲选法分为参照组和观察组,各42例。参照组实施锁定钢板内置术治疗,观察组实施外治钢板加VAD治疗,比较两组患者骨折愈合优良率、创面愈合时间、骨折愈合时间、住院时间及术后并发症发生率。结果研究组骨折愈合优良率为92.86%,高于参照组的76.19%,差异有统计学意义(P<0.05);研究组患者创面愈合时间、骨折愈合时间、住院时间均短于参照组,差异有统计学意义(P<0.05);研究组术后并发症发生率为11.90%,低于参照组的30.95%,差异有统计学意义(P<0.05)。结论外置钢板加VSD在治疗软组织严重损伤的胫骨开放粉碎性骨折中的应用效果显著,可促进患者创面愈合及骨折愈合,且术后并发症少,值得临床推广运用。Objective To investigate the application value of external steel plate and closed negative pressure drainage(VAD)in the treatment of severely injured tibial open comminuted fractures.Methods 84 patients with open comminuted fractures of the tibia who were admitted to our hospital from May 2018 to May 2020 were selected and they were divided into the reference group and the observation group by computer blind selection method,with 42 cases in each group.The reference group was treated with locking plate internal surgery,and the observation group was treated with external treatment plate plus VAD.The excellent rate of fracture healing,wound healing time,fracture healing time,hospital stay and postoperative complications between two groups were compared.Results The excellent rate of fracture healing in the study group was 92.86%,which was higher than 76.19%in the reference group,the difference was statistically significant(P<0.05).The wound healing time,fracture healing time,and hospital stay of the study group were shorter than those in the reference group,the difference was statistically significant(P<0.05);the postoperative complication rate in the study group was 11.90%,which was lower than of 30.95%in the reference group,the difference was statistically significant(P<0.05).Conclusion The application of external steel plate and VSD in the treatment of severely injured tibia open comminuted fractures is effective.It can promote wound healing and fracture healing in patients with few postoperative complications.It is worthy of clinical application.
