医养护一体化管理模式在社区老年慢性病患者管理中的应用

摘　　要：目的评价在社区老年慢性病患者管理中医养护一体化管理模式的应用效果。方法2018年8月至2019年12月,抽取本院收治的本社区纳入公共卫生管理的慢性病老年患者100例作为研究对象,按照计算机随机分组法,将100例患者分为对照组、试验组。对照组(n=50)用常规管理模式,试验组(n=50)用医养护一体化管理模式,对比护理效果。结果试验组自我护理能力评分明显高于对照组(P<0.05),安全用药知识掌握情况明显高于对照组(P<0.05)。结论在社区老年慢性病患者管理中应用医养护一体化管理模式,可提高患者自我护理能力、安全用药知识掌握情况。Objective To evaluate the application effect of the integrated management model of traditional Chinese medicine and nursing in the management of elderly patients with chronic diseases in the community.Methods From August 2018 to December 2019,one hundred elderly patients with chronic diseases who were admitted to our community for public health management were selected as the research object,and 100 patients were divided into a control group and an experimental group by computer random grouping.The control group(n=50)used the conventional management model,and the experimental group(n=50)used the integrated medical and nursing management model to compare the nursing effects.Results The self-care ability score of the experimental group was significantly higher than that of the control group(P<0.05),and the knowledge of safe medication was significantly higher than that of the control group(P<0.05).Conclusion In the management of elderly patients with chronic diseases in the community,the application of integrated medical and nursing management mode can improve the patient's self-care ability,knowledge of safe medication,and can also improve medication compliance.
