电视胸腔镜双侧肺减容术治疗慢性阻塞性肺气肿效果

摘　　要：目的分析电视胸腔镜双侧肺减容术治疗慢性阻塞性肺气肿的应用效果。方法方便选取该院2019年5月—2020年5月诊治的78例慢性阻塞性肺气肿患者开展该次试验研究,将所有患者随机分为参照组39例和研究组39例。参照组行常规开胸肺减容术治疗,研究组行电视胸腔镜双侧肺减容术治疗,比较两组临床疗效。结果研究组的PaO_(2)指标(89.31±5.32)mmHg、6MWT指标(433.42±68.65)m和PaCO_(2)指标(31.35±4.25)mmHg均明显优于参照组的(80.36±4.25)mmHg、(405.22±41.11)m和(37.26±4.21)mmHg,差异有统计学意义(t=8.208、2.201、6.170,P<0.05);研究组并发症发生率5.13%明显低于参照组20.51%,差异有统计学意义(χ^(2)=4.129,P=0.042)。结论给予慢性阻塞性肺气肿患者电视胸腔镜双侧肺减容术治疗能够显著改善患者的相关指标变化情况,治疗安全性高。Objective To analyze the application effect of video-assisted thoracoscopic bilateral lung volume reduction surgery in the treatment of chronic obstructive pulmonary emphysema.Methods A total of 78 patients with chronic obstructive pulmonary emphysema diagnosed and treated in the hospital from May 2019 to May 2020 were convenient selected to carry out this experimental study.All patients were randomly divided into a reference group of 39 cases and a study group of 39 cases.The reference group was treated with conventional thoracotomy for lung volume reduction,and the study group was treated with video-assisted thoracoscopic bilateral lung volume reduction surgery to compare the clinical effects of the two groups.Results The PaO_(2) index(89.31±5.32)mmHg,6MWT index(433.42±68.65)m and PaCO_(2) index(31.35±4.25)mmHg of the study group were significantly better than those of the reference group(80.36±4.25)mmHg and(405.22±41.11)m and(37.26±4.215)mmHg,and the difference was statistically significant(t=8.208,2.201,6.170,P<0.05);the complication rate of the study group was 5.13%was significantly lower than that of the reference group 20.51%,and the difference was statistically significant(χ^(2)=4.129,P=0.042).Conclusion Video-assisted thoracoscopic bilateral lung volume reduction surgery for patients with chronic obstructive pulmonary emphysema can significantly improve the changes in related indicators of patients,and the treatment is safe and has promotion value.
