股骨颈骨折患者经外侧入路行人工全髋关节置换术的临床疗效探究

摘　　要：目的探究股骨颈骨折患者经外侧入路行人工全髋关节置换术的临床疗效。方法 50例股骨颈骨折患者,均经外侧入路行人工全髋关节置换术。分析患者的临床指标(术中出血量、切口长度、手术时间、术后第1天的引流量);术后3、6、12个月的Harris评分,优良率, SF-36评分;术中及术后的并发症发生情况。结果 50例患者的术中出血量为(372.13±68.93)ml、切口长度为(9.52±1.12)cm、手术时间为(93.01±12.03)min、术后第1天引流量为(110.19±9.32)ml。术后3、6、12个月,患者的Harris评分分别为(77.23±3.26)、(84.35±4.36)、(96.32±2.36)分。随着时间的延长,患者的Harris评分逐渐升高,比较差异具有统计学意义(P<0.05)。术后9个月,患者的优良率为94%,高于术后3个月的50%及术后6个月的66%,差异具有统计学意义(P<0.05)。术后3个月与术后6个月的优良率比较,差异无统计学意义(P>0.05)。术后3、6、12个月,患者的SF-36评分分别为(66.32±5.36)、(78.63±6.35)、(89.65±4.83)分。随着时间的延长,患者的SF-36评分逐渐升高,比较差异具有统计学意义(P<0.05)。患者术中及术后均未出现切口感染、深静脉血栓、神经血管损伤、假体松动或脱位等并发症。结论股骨颈骨折患者经外侧入路行人工全髋关节置换术的手术效果好,术后功能恢复快,安全性好,是一种适合临床的理想治疗方法。Objective To investigate the clinical efficacy of total hip arthroplasty through lateral approach in patients with femoral neck fracture.Methods 50 patients with femoral neck fractures all underwent total hip arthroplasty via lateral approach.The clinical indicators(intraoperative blood loss,incision length,operation time and drainage volume on the first day after operation),Harris score at 3,6 and 12 months after operation,excellent rate,SF-36 score,and occurrence of complications during and after the operation were analyzed.Results The intraoperative blood loss of 50 patients was(372.13±68.93)ml,incision length was(9.52±1.12)cm,operation time was(93.01±12.03)min and drainage volume on the first day after operation was(110.19±9.32)min.At 3,6 and 12 months after operation,the Harris score were(77.23±3.26),(84.35±4.36)and(96.32±2.36)points.As time went by,the Harris score of the patient gradually rise,and the difference was statistically significant(P<0.05).At 9 months after operation,the excellent rate was 94%,which was higher than 50%at 3 months after operation and 66%at 6 months after operation.All the difference was statistically significant(P<0.05).There was no statistically significant difference in excellent rate at 3 and 6 months after operation(P>0.05).At 3,6 and 12 months after operation,the SF-36 score were(66.32±5.36),(78.63±6.35)and(89.65±4.83)points.As time went by,the SF-36 score gradually rise,and the difference was statistically significant(P<0.05).There were no complications such as wound infection,deep vein thrombosis,neurovascular injury,prosthesis loosening or dislocation during and after the operation.Conclusion Total hip arthroplasty vis lateral approach shows good operation effect on femoral neck fracture patients with quick functional recovery and good safety,which is an ideal treatment method for clinical treatment.
