外固定架结合中频脉冲疗法治疗C型桡骨远端骨折患者血清炎症因子和氧化应激指标的变化

摘　　要：背景:C型桡骨远端骨折常采用外固定支架固定治疗,但仍有部分患者存在骨折愈合延迟、骨痂生长缓慢等不良症状发生,术后如何促进骨折愈合及腕关节功能恢复具有重要意义。目的:探讨外固定架结合中频脉冲疗法治疗C型桡骨远端骨折的疗效及对氧化应激及炎性递质的影响。方法:选取广东省人民医院2018年1至12月收治的86例C型桡骨远端骨折患者的临床资料进行回顾性分析,按照不同治疗方式分为2组,外固定支架组41例,联合组(中频脉冲疗法联合外固定支架治疗)45例。治疗后4周观察两组患者的临床疗效;对比两组患者治疗前及治疗4周后血清中炎症递质、氧化应激指标水平的变化。结果与结论:①治疗后4周,联合组骨痂量化评分高于外固定支架组,目测类比评分及Gartland-Werley评分低于外固定支架组(P<0.05);②治疗后4周,两组患者丙二醛、晚期氧化蛋白产物水平均下降,且联合组低于外固定支架组(P<0.05);抗氧化物谷胱甘肽过氧化物酶、超氧化物歧化酶水平均升高,且联合组高于外固定支架组(P<0.05);③治疗后4周,两组血清中C-反应蛋白、白细胞介素6、白细胞介素1β的水平均较治疗前显著降低(P<0.05),且联合组低于外固定支架组(P<0.05);④提示中频脉冲疗法结合外固定支架可促进C型桡骨远端骨折愈合及腕关节功能恢复,降低术后炎症及氧化应激反应。BACKGROUND:Type C distal radius fractures are often treated with external fixation.However,some patients still have undesirable symptoms such as delayed fracture healing and slow growth of callus.How to promote fracture healing and recovery of wrist joint function after surgery is of great significance.OBJECTIVE:To investigate the effect of mid-frequency pulse therapy combined with external fixator on type C distal radius fracture and its influence on oxidative stress and inflammatory mediators.METHODS:Clinical data of 86 patients with type C distal radius fracture admitted in Guangdong Provincial People’s Hospital from January to December 2018 were analyzed retrospectively.Patients were divided into two groups according to different treatment methods:external fixation group(n=41 cases)and combined group(mid-frequency pulse therapy combined with external fixation treatment,n=45 cases).The clinical effect of different treatment methods was observed,and the changes of serum inflammatory mediators and oxidative stress indexes before and 4 weeks after treatment were compared between the two groups.RESULTS AND CONCLUSION:(1)The quantitative score of callus in the combined group was higher than that in the external fixation group,and the visual analogue scale score and Gartland-Werley score were lower than that in the external fixation group(P<0.05).(2)At 4 weeks after treatment,malondialdehyde and advanced oxidation protein product levels decreased,and the level in the combined group was lower than that in the external fixation group(P<0.05).The level of antioxidant glutathione peroxidase and superoxide dismutase levels increased;the level in the combined group was higher than that in the external fixation group(P<0.05).(3)At 4 weeks after treatment,the levels of C-reactive protein,interleukin-6,and interleukin-1βin the serum of the two groups were significantly lower than those before treatment(P<0.05),and the levels in the combined group were lower than those in the external fixation group(P<0.05).(4)It is conclu
