摘  要:目的研究重组人角质细胞生长因子-2对重度烟雾吸入性损伤兔肺组织的作用。方法40只普通大白兔烟雾吸入性损伤模型,并根据雾化吸入不同剂量KGF-2分成PBS组和1、3、5mg/kg组,每组均1次/30min/d,并于治疗4d后进行动脉血气分析;检测肺组织中肺表面活性蛋白A(SP-A)、血管内皮生长因子(VEGF)、Bcl-2 mRNA及蛋白的表达水平,并观察治疗后肺部组织的病理结构。结果与PBS组比较,随着rhKGF-2的剂量的增加,PaO_(2)也是逐渐升高,5mg/kg组PaO_(2)升高最为明显(P<0.01);与PBS组比较,当药物剂量为1mg/kg时,兔子PaCO_(2)有降低的趋势,随着rhKGF-2剂量的增加,PaCO_(2)也随之升高,当药物剂量为5mg/kg时,PaCO_(2)升高最明显;雾化吸入rhKGF-2后,不同剂量组肺部组织病理结果不同,高剂量组中,肺部组织损伤较轻,其修复效果较为明显。并且肺组织中Bcl-2、SP-A、VEGF mRNA表达变化明显,呈现增加的趋势,其中药物为5mg/kg时,变化最为明显,其蛋白表达的增强均与药物浓度相关。结论雾化吸入KGF-2可以增加烟雾损伤模型兔肺组织的氧合作用,减轻肺部损伤,且随着KGF-2药量的增加,其修复效果越好。Objective To study the effect of recombinant human keratinocyte growth factor-2(rhKGF-2)on lung tissue of rabbits with severe smoke inhalation injury.Methods 40 normal rabbits were divided into the PBS group and the 1,3,5 mg/kg groups according to different doses of KGF-2 inhalation,once/30 min/D in each group.Arterial blood gas analysis was performed 4 days after treatment.Pulmonary surfactant protein A(SP-A),vascular endothelial growth factor(VEGF)and Bcl-2 in lung tissue were detected.The expression levels of mRNA and protein,and the pathological structure of lung tissue after treatment were observed.Results Compared with the PBS group,PaO_(2)increased gradually with the increase of rhKGF-2 dose,and it was the most pronounced in the 5mg/kg group.Compared with the PBS group,PaCO_(2)increased significantly in the group A(P<0.01).Compared with the PBS group,PaCO_(2)decreased with the increase of rhKGF-2 dose,and the increase of PaCO_(2)was the most obvious when the dose of rhKGF-2 was 5mg/kg.After inhalation of rhKGF-2,the pathological results of lung tissue in different dose groups were different,and the lung tissue damage in the high-dose group was lighter than that in the PBS group.The effect was obvious.The expression of Bcl-2,SP-A,VEGF mRNA in lung tissue changed significantly,showing an increasing trend,especially when the dose was 5mg/kg.The increase of protein expression was related to drug concentration.Conclusion Aerosol inhalation of KGF-2 can increase the oxygenation of lung tissue in smoke injury model rabbits,and reduce lung injury.With the increase of KGF-2 dosage,the repair effect is better.
