摘 要:目的探讨宫颈癌后装腔内照射不同施源器放置技术与剂量分布对于效果和不良反应发生率的影响。方法选取2016年5月—2018年5月于医院进行治疗的宫颈癌患者60例,随机分为两组,在体外三维行适型调强放疗基础上,研究组选用三管施源器(Fletch),对照组选用单管施源器,对比放疗8周后两组患者临床疗效、生存时间以及不良反应发生情况的差异。结果干预后研究组包容宫颈、膀胱、直肠的体积显著优于对照组(P<0.05);放疗后随访至今,治疗后研究组生存总时间及无瘤生存时间较对照组更长(P<0.05);治疗后两组患者不良反应发生发生率为6.7%(研究组)和30.0%(对照组),研究组整体发生率较对照组更低(P<0.05)。结论采用三管放置施源器放置技术治疗宫颈癌效果显著,在减少对周边器官组织的损伤的同时,可以对靶区剂量进行有效控制,降低不良反应的发生率,整体疗效显著,可有效延长患者生存期。Objective To explore the effects of different applicator placement techniques and dose distribution on the effect and incidence of adverse reactions after intracavity irradiation of cervical cancer.Methods Sixty cervical cancer patients treated in our hospital from May 2016 to May 2018 were selected and randomly divided into two groups.On the basis of three-dimensional in-vitro intensity-modulated radiation therapy in vitro,the research group was used a three-tube applicator(Fletch),the control group was used a single-tube applicator to compare the differences in clinical efficacy,survival time,and adverse reactions between the two groups after 8 weeks of radiotherapy.Results After intervention,the volume of cervical,bladder and rectum included in the study group was significantly better than that of the control group(P<0.05),follow-up after postoperative radiotherapy so far,the total survival time and tumor-free survival time of the study group after treatment were longer than those of the control group(P<0.05),after treatment,the incidence of adverse reactions in the two groups of patients was 6.7%(study group)and 30.0%(control group),the overall incidence rate of the study group was lower than that of the control group(P<0.05).Conclusion The three-tube placement applicator placement technology has a significant effect on cervical cancer.While reducing damage to surrounding organs and tissues,it can effectively control the dose in the target area and reduce the incidence of adverse reactions. The overallefficacy is significant and effectively extend patient survival.
