湿热痹颗粒辅助苯溴马隆片对痛风性关节炎患者血清免疫、炎症指标表达的影响

摘　　要：目的探究湿热痹颗粒辅助苯溴马隆片治疗痛风性关节炎(Gouty arthritis,GA)的临床疗效。方法按照随机数字表法将90例GA患者分为对照组与治疗组,每组45例。两组均给予常规治疗与干预,在此基础上,对照组给予苯溴马隆片,治疗组给予湿热痹颗粒辅助苯溴马隆片治疗,两组均治疗2周。对比两组临床疗效、安全性,治疗前后关节疼痛(VAS评分)、血尿酸水平及血清免疫炎症指标[干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)]水平变化、生活质量(SF-12)评分改善情况。结果治疗组治疗后治疗总有效率高于对照组(P <0.05);治疗后两组VAS评分均较本组治疗前降低,且治疗组低于对照组(P <0.05);治疗后两组血尿酸水平均较本组治疗前降低,且治疗组低于对照组(P <0.05);治疗后两组血清IFN-γ、IL-4水平均较本组治疗前改善,且治疗组优于对照组(P <0.05);治疗后两组SF-12评分均较本组治疗前增加,且治疗组高于对照组(P <0.05);两组不良反应发生率比较,差异无统计学意义(P> 0.05),且均评定为2级,未影响治疗。结论湿热痹颗粒辅助苯溴马隆片治疗GA安全、有效,可有效降低血尿酸水平,缓解关节疼痛,改善患者生活质量,且可能与调节血清免疫、炎症指标表达有关。Objective To explore the clinical value of Shirebi Granules as an adjunct to benzbromarone tablets in the treatment of gouty arthritis( GA). Method According to the random number table method,90 GA patients in our hospital were divided into control group and treatment group,45 in each group. Both groups were given routine treatment and intervention,on this basis,the control group was given benzbromarone tablets,and the treatment group was given damp-heat Bi granules assisted benzbromarone tablets,both groups were treated for 2 weeks. The clinical efficacy and safety,joint pain( VAS score),serum uric acid level,serum immunoinflammatory indicators[interferon-gamma( IFN-γ),interleukin-4( IL-4) ]level changes and quality of life( SF-12 score) improvement before and after treatment were compared between the two groups. Result( 1) Clinical efficacy: the total effective rate of the treatment group was 95. 56% higher than that of the control group( 83. 72%( P < 0. 05).( 2) Joint pain: after treatment,VAS scores of both groups were lower than before treatment,and the treatment group was lower than the control group( P < 0. 05).( 3) Serum uric acid level: After treatment,the serum uric acid level in both groups was lower than before treatment,and the treatment group was lower than the control group( P < 0. 05).( 4) Immune inflammation index: After treatment,serum IFN-gamma and IL-4 levels in both groups were improved compared with those before treatment,and the treatment group was better than the control group( P < 0. 05).( 5) Quality of life: after treatment,the SF-12 scores of both groups increased compared with before treatment,and the treatment group was higher than the control group( P < 0. 05).( 6) Safety: There was no significant difference in the incidence of adverse reactions between the two groups( P > 0. 05),and both groups were rated as grade 2,which did not affect the treatment.
