摘　　要：目的探讨封闭负压引流联合微动力负压敷料促进Ⅱ-Ⅳ级糖尿病足创面愈合的有效性及价值。方法将60名患者分为两组,对照组采用常规治疗方法+封闭负压引流+普通换药及敷料覆盖,观察组采用常规治疗方法+封闭负压引流+微动力负压敷料治疗。观察治疗情况(目标溃疡完全愈合率、溃疡面积缩小率、溃疡完全愈合时间、换药次数、最后1个月随访期间溃疡复发率),不良事件发生情况(出血、疼痛、红肿)。结果观察组愈合率(100%)高于对照组(93.3%);创面缩小率高于对照组(P<0.05);愈合时间、换药次数及1个月复发率均低于对照组(P<0.05)。观察组不良反应发生率(16.7%)低于对照组(46.7%)(P<0.05)。结论封闭负压引流联合微动力负压敷料促进Ⅱ-Ⅳ级糖尿病足创面治疗中效果显著,并且不良反应少,可促进肉芽组织及上皮生长,加速伤口愈合。Objective To explore the effectiveness and value of vacuum sealing drainage combined with micro⁃dynamic vacuum pack dressing in accelerating healing of the wounds in gradeⅡ~Ⅳdiabetic foot.Methods 60 patients were divided into two groups.The control group received conventional treatment,vacuum sealing drainage,ordinary dressing change,and dressing coverage;while the study group received conventional treatment,vacuum sealing drainage,and micro⁃dynamic vacuum pack dressing.The treatment situation including target ulcer complete healing rate,ulcer area reduction rate,ulcer complete healing time,number of dressing changes,and ulcer recurrence rate during the last one month follow⁃up,and adverse events such as bleeding,pain,redness and swelling were observed.Results The healing rate was greater in the study group(100%)than in the control group(93.3%);the wound reduction rate was greater(P<0.05),the healing time was shorter,the number of dressing changes was smaller,and the one⁃month recurrence rate were lower in the study group(P<0.05).The incidence of adverse reactions in the study group(16.7%)was lower than that in the control group(46.7%)(P<0.05).Conclusions Vacuum sealing drainage combined with micro⁃dynamic vacuum pack dressing has a significant effect in the treatment ofⅡ~Ⅳdiabetic foot wounds and has fewer adverse reactions.It may boost growth of granu⁃lation tissue and epithelium and accelerate wound healing.
