摘　　要：目的建立适合武汉地区成人的血清细胞角蛋白19片段(CYFRA21-1)参考区间,并初步探讨影响CYFRA21-1水平的因素。方法选取符合条件的健康体检者8199名,检测其血清CYFRA21-1和血清肌酐(SCr)水平,并计算估算肾小球滤过率(eGFR)。采用非参数法,取单侧第95百分位数(P_(95))值确定参考区间上限。分析性别、年龄、eGFR对CYFRA21-1的影响。结果根据Dixon法结果剔除7个离群值,共纳入8192名健康体检者。男性血清CYFRA21-1水平明显高于女性(P<0.001)。不同年龄组之间CYFRA21-1水平差异有统计学意义(P<0.001)。男性、女性血清CYFRA21-1水平的P_(95)值均随年龄的增长而升高,≥60岁各年龄组明显升高。采用非参数法确定参考区间:<60岁为0~3.37 ng/mL,60~69岁为0~4.17 ng/mL,≥70岁为0~5.67 ng/mL。Spearman秩相关分析结果显示,血清CYFRA21-1水平与年龄呈正相关(r=0.213,P<0.001),与eGFR呈负相关(r=-0.247,P<0.001),eGFR与年龄呈负相关(r=-0.612,P<0.001)。分位数回归分析结果显示,年龄(tau=0.95,β=0.02,P<0.001)、性别(tau=0.95,β=-0.24,P<0.001)和eGFR(tau=0.95,β=-0.01,P<0.001)均是CYFRA21-1水平的影响因素。结论建立了适用于武汉地区成人的CYFRA21-1参考区间。性别、年龄和eGFR均是血清CYFRA21-1水平的影响因素。Objective To establish the reference interval of serum cytokeratin 19 fragment(CYFRA21-1)for local population,and to preliminarily investigate the factors for the level of CYFRA21-1.Methods A total of 8199 healthy subjects were enrolled.Serum CYFRA21-1 and serum creatinine(SCr)levels were determined,and the estimated glomerular filtration rate(eGFR)was calculated.Non-parametric method was used to determine the upper limit of the reference interval by taking the unilateral 95th percentile(P_(95)).The influence factors of sex,age and eGFR on CYFRA21-1 were analyzed.Results The Dixon method was used to exclude 7 outliers from the 8199 subjects,and a total of 8192 subjects were enrolled.Serum CYFRA21-1 level in males was higher than that in females(P<0.001).There was statistical significance in CYFRA21-1 levels in different age groups(P<0.001).The P_(95) of serum CYFRA21-1 levels in males and females increased with age,especially in≥60 years old groups.Non-parametric method was used to determine the reference intervals,which were 0-3.37 ng/mL for<60 years old,0-4.17 ng/mL for 60-69 years old,0-5.67 ng/mL for≥70 years old.Spearman rank correlation analysis showed that serum CYFRA21-1 levels were positively correlated with age(r=0.213,P<0.001),and they were negatively correlated with eGFR(r=-0.247,P<0.001).The eGFR was negatively correlated with age(r=-0.612,P<0.001).Quantile regression analysis showed that age(tau=0.95,β=0.02,P<0.001),sex(tau=0.95,β=-0.24,P<0.001)and eGFR(tau=0.95,β=-0.01,P<0.001)were influence factors of CYFRA21-1 level.Conclusions The CYFRA21-1 reference interval in Wuhan has been established.Sex,age and eGFR are the influencing factors for serum CYFRA21-1 level.
