摘 要:目的:探讨饮水、静脉注射小剂量呋塞米对CT尿路造影(CTU)中正常上尿路显示质量的影响。方法:120例患者按照检查顺序随机分为4组,即不饮水不静注呋塞米组(A组)、饮水不静注呋塞米组(B组)、不饮水静注呋塞米组(C组)、饮水并静注呋塞米组(D组),饮水患者于扫描前30min饮水10ml/kg,静注呋塞米患者于检查前5min静注10mg呋塞米。将上尿路分为4段,即肾内集合系统、输尿管近段、输尿管中段和输尿管远段,分别比较4组各段上尿路腔内的平均CT值、扩张度及连续性评分的差异。结果:各组4段上尿路CT值差异有统计学意义(P<0.05),A组CT值最高,D组最低,B、C组差异无统计学意义(P>0.05)。各组4段尿路扩张度及连续性评分差异有统计学意义(P<0.05),A组最高,B、C、D组间差异均无统计学意义(P>0.05)。结论:饮水、小剂量呋塞米可降低上尿路各段的CT值,提高了各段的扩张度及连续性,联合饮水和小剂量呋塞米虽然能进一步降低各段CT值,但未能提高各段扩张度及连续性。Objective:To explore the influence of drinking water and low dose furosemide on the visualization of normal upper urinary tract during the M DCTU.Methods:120 patients were randomly divided into 4 groups according to the order of examination,namely the non-drinking nor intravenous furosemide group(group A),the non-drinking nor intravenous furosemide group(group B),and the non-drink ing intravenous furosemide group(group C),drinking water and intravenous furosemide group(group D).The water-drinking patients drank 10 ml/kg 30 minutes before the scan,and the intravenous furosemide patients received 10 mg furosemide intravenously 5 minutes before the examination.The upper tract was divided into four segments,the renal collecting system,the distal ureter,the middle ureter and the lower ureter.The mean CT value,expansion and continuity score of urinary tracts in four groups were compared.Results:There was statistically significant difference in the four segments of CT value.CT value was highest in group A and lowest in group D.There was no statistically significant difference between group B and C.There was statistically significant difference in all segments of the four groups of expansion and continuity score.Group A was the highest.There was no statistically significant difference among group B,C and D.Conclusion:Drinking water and low-dose of furosemide can reduce the CT value of each segment of the urinary tract and improve the expansion and continuity of each segment.Although the combination of drinking water and low dose of furosemide can further reduce the CT value of each segment,it fails to improve the expansion and continuity of each segment.
