不同皮瓣修复术在手部创面修复中的应用效果及对患者满意度的影响

摘　　要：目的研究不同皮瓣修复术在手部创面修复中的应用效果及对患者满意度的影响。方法选取我院2018年2月至2020年2月收治的124例手部创面修复患者作为研究对象,采用随机数字法将其分为对照组和观察组,各62例。对照组患者采用游离腹壁下动脉穿支皮瓣修复术治疗,观察组患者采用微型游离腓动脉穿支皮瓣修复术治疗。比较两组患者的血清促炎细胞因子水平、手部创面修复效果、感染发生情况及患者术后满意度。结果手术后,两组患者血清IL-6、IL-8、IFN-γ、TNF-α水平均高于手术前,但观察组低于对照组,差异具有统计学意义(P<0.05)。观察组手部创面修复总有效率为96.77%,高于对照组的82.26%(P=0.01)。观察组手术后创面总感染率为1.61%,低于对照组的11.29%(P=0.03)。观察组术后满意度为98.39%,高于对照组的83.87%(P=0.00)。结论游离腹壁下动脉穿支皮瓣和微型游离腓动脉穿支皮瓣均能作为手部创面有效的修复手术,但后者更能有效抑制炎症反应程度,降低感染风险,有助于术后恢复且患者满意度高。Objective To study the application effect of different skin flap repairs in hand wound repair and its influence on patients'satisfaction.Methods A total of 124 patients with hand wound repair admitted in our hospital from February 2018 to February 2020 were selected as the research objects,and the patients were divided into control group and observation group by random number method,with 62 cases in each group.The control group was treated with free inferior abdominal artery perforator flap repair,and the observation group was treated with micro free peroneal artery perforator flap repair.The levels of serum pro-inflammatory cytokines,repair effect of hand wound surface,infection and postoperative satisfaction of patients were compared between the two groups.Results After operation,the levels of serum IL-6,IL-8,IFN-γand TNF-αin the two groups were higher than those before operation,but those in the observation group were lower than the control group,and the differences were statistically significant(P<0.05).The total effective rate of hand wound repair in the observation group was 96.77%,which was higher than 82.26%in the control group(P=0.01).The total wound infection rate of the observation group after operation was 1.61%,which was lower than 11.29%of the control group(P=0.03).The satisfaction of patients after operation in the observation group was 98.39%,which was higher than 83.87%in the control group(P=0.00).Conclusion Both free inferior abdominal artery perforator flap and micro free peroneal artery perforator flap can be used as effective repair operations for hand wounds,the latter can more effectively inhibit the degree of inflammatory reaction,reduce the risk of infection,contribute to postoperative recovery and has high patients'satisfaction.
