某医院门诊处方抗菌药物不合理应用情况及对策分析

摘　　要：目的调查分析某医院门诊处方抗菌药物不合理应用情况,并探讨促进门诊处方抗菌药物合理应用的对策。方法抽取2019年1月至2019年12月我院门诊应用抗菌药物的1000张处方进行回顾性研究。统计抗菌药物不合理应用情况,并分析抗菌药物不合理应用所致不良反应的发生特点。结果1000张抗菌药物处方的处方不合格率为9.80%(98/1000);不合理应用情况以药物配伍不合理、无明确指征用药为主;不合理用药科室主要为普外科、呼吸内科、消化内科、妇产科。不良反应发生率为61.22%(60/98),年龄集中于≥60岁人群;抗菌药物种类为头孢菌素类、青霉素类、喹诺酮类、大环内酯类;不良反应累及系统以皮肤、消化系统为主,症状表现以皮疹、恶心、呕吐为主。结论医院门诊处方中抗菌药物不合理应用情况不容乐观,易引发药物不良反应,临床上应根据抗菌药物不合理应用情况实施针对性干预。Objective To investigate and analyze the irrational use of antibacterial drugs in outpatient prescriptions of a hospital,and to explore the countermeasures to promote the rational use of antibiotics in outpatient prescriptions.Methods From January 2019 to December 2019,1000 prescriptions of antibacterial drugs used in the outpatient department of our hospital were selected for retrospective study.The irrational use of antibacterial drugs was counted and the characteristics of adverse reactions caused by the irrational use of antibacterial drugs were analyzed.Results The unqualified rate of 1000 prescriptions of antibacterial drugs was 9.80%(98/1000);the irrational use of drugs was mainly irrational drug compatibility and medication without clear indications;the main departments of irrational drug use were general surgery department,respiratory department,digestive department and obstetrics and gynecology department.The incidence of adverse reactions was 61.22%(60/98),and the age was concentrated in people≥60 years old;the types of antibacterial drugs were cephalosporins,penicillins,quinolones,and macrolides;the adverse reactions mainly involved skin and digestive system,and the main symptoms were rash,nausea and vomiting.Conclusion The irrational use of antibacterial drugs in outpatient prescriptions in hospital is not optimistic,it is easy to cause adverse drug reactions.Therefore,targeted intervention should be implemented according to the irrational use of antibiotics in clinic.
