摘　　要：目的:探讨急性肾损伤(AKI)患者肾功能未恢复的影响因素,评估血镁水平对肾脏预后的预测价值.方法:分析110例AKI患者的临床资料,根据确诊AKI后90d肾功能恢复情况分为恢复组和未恢复组,比较两组患者的临床指标,用Logistic回归分析AKI患者肾功能未恢复的独立危险因素,建立临床预测模型,绘制ROC曲线,评估各独立危险因素及临床模型对AKI患者肾脏预后的预测价值.结果:多因素Logistic回归分析显示,低镁血症、贫血、高龄、出院时高血清肌酐(SCr)是AKI患者确诊后90d肾功能未恢复的独立危险因素.ROC分析显示,低镁血症对AKI肾脏预后具有预测价值,同时联合独立危险因素及尿素的多变量模型对AKI肾脏预后预测效能较高(AUC=0.944,P<0.001).结论:血清镁有助于预测AKI患者的肾脏预后,由低镁血症、贫血、年龄等因素建立的临床模型对AKI患者肾脏预后具有较高的预测价值.Objective:To investigate the risk factors of renal function in patients with acute kidney injury(AKI),and to evaluate the predictive value of serum magnesium for renal prognosis.Methodology:The clinical data of 110 patients with AKI were analyzed.According to the renal function 90 days after diagnosis of AKI,the patients were divided into recovery group and non recovery group,the clinical indexes of the two groups were compared.The independent risk factors of non recovery of renal function in AKI patients were analyzed by logistic regression.The clinical risk factor model was established,and the receiver operating characteristic curve(ROC)was drawn to evaluate the independent risk factors objective to investigate the predictive value of risk factors and clinical models for renal prognosis in AKI patients.Results:Multivariate logistic regression analysis showed that hypomagnesemia,anemia,age and high serum creatinine at discharge were independent risk factors for non recovery of renal function 90 days after diagnosis of AKI.ROC curve showed that hypomagnesemia had predictive value for AKI renal prognosis,and multivariate model combined with independent risk factors and urea had higher predictive value for AKI renal prognosis(AUC=0.944,P<0.001).Conclusion:Serum magnesium is helpful to predict the prognosis of AKI patients.The clinical model established by hypomagnesemia,anemia,age,discharge SCr and urea has a high predictive value for the prognosis of AKI patients.
